| Vascular dementia(VD) is a kind of dementia syndroms caused by cerebral vascular factors that cause substantial damage to mental impairment in at least three of the following : language, memory, visual spatial skills, emotion, personality and other cognitive function (such as calculation, abstract judgment),.It Is a disease of middle-aged and elderly one.Before 1970's,Vascular dementia was adopted ischemia dementia in the.western.countries.Until1974,Hachiski ect confirmed by histopathology that not only bilateral hemispheric ischemic infarction resulted in declination of cognitive function,chunks or the important parts of a single or a small infarction also leaded to the incidence of dementia.And hemorrhagic or acute and chronic hypoxic-ischemic encephalopathy was the cause of cognitive function.With the development of science,in 1992. World Health Organization (WHO) promulgated the "separation barrier ICD210 spirit and action," the unification of vascular dementia was named, and that has been widely recognized by the medical profession.With the development of modern imaging technology,gene technology and histopathology ,It has been realized deeply that vascular dementia is the result of many common machanism of degenerative vascular factors and other factors.Vascular factors play a leading role in all of them.Abnormal neurotransmitters, immune dysfunction,inflammatory response,gene mutation that associated with VD ,which caused by the various reasons. are likely to be involved in the occurrence of VD,Moreover, it has beenproved that VD is one of the diseases that proboblely can be prevented and treated and is so far the only type of dementia that can be prevented.So it has become the focus of the study of the more and more numerous scholars.Because of the complex etiology and pathogenesis of vascular dementia, there is no theory that can explain satisfactorily its real mechanism yet.Although to prevent the recurrence of stroke is the key to this treatment, it is an important component of the treatment of VD to improve the brain function and to enhance memory and cognitive abilities to help restore patients. At present ,There are a lot of drugs to improve brain function in patients with VD.Hydergine (hydergine) is a mesylate agents.It can block alpha-adrenergic receptor to expand cerebral vascular. And it has been improved that it is effective to treat vascular dementia.In the clinical studies, it is currently recognized one of the drugs in the treatment of vascular dementia in our courtry.With the gradual increase in life and an aging population, the incidence of vascular dementia is rising year by year .In China, the treatment of vascular dementia has been incorporated into the Ninth Five-key project. Congzhi granules are composed by a variety of Chinese medicine on the basis of the treatment of vascular dementia of Academician Yongyan and Professor Zhang Boli in Chinese medicine for many years and after clinical and experimental research of the expert group. Congzhi granules are one of the prescriptions of the Five-key project "The Effect of Yishenhuazhuo treatment of vascular dementia in the elderly of the treatment of VD,by the State Science and Technology Commission.The study is a part of the Ninth Five-research project, whichaimed at the efficiency and safety of Congzhi granules to vascular dementia, and taking into account the values and aspirations of patients,with the the scientific, rigorous clinical research and the professional skills and personal clinical experience of my tutor, with a focus on the clinical symptoms. In clinical data, the diagnostic criteria of the American Academy of Neurology "Diagnostic and Statistical Manual of Neurology "4 (DSM-IV) criteria.was adopted ,whichwas announced officially by the World Health Organization (WHO).Neuropsychological tests mini-mental state examination (MMSE) score and Blessed Behavior Checklist (BBS) scores were used to evaluate the effect in clinical studies. Eighty cases were completed in the clinical study ,which are divided in random into Congzhi granules treatment group (treatment group) 59 cases and Hydergine treatment (control group), 21 cases.In the research,cognitive function, normal behavior and safety has been on the focus for a further objective assessment.Eighty cases were completed in the clinical study .The results showed that in this study, the average age, sex, disease course, education and the degree of dementia, neuropsychological tests scores, risk factors and image date of the two groups were consistent with the clinical features of VD.In clinical efficacy,MMSE scores of patients after treatment were significantly increased than before(P<0.05). The cognitive function efficiency of the treatment group was 72.88%,71.43% of the control group,the difference between the two groups was not statistically significant (P>0.05). Orientation, delayed memory, attention, calculation and language of the patients in the treatment group were improvedsignificantly. After treatment, the scores of BBS dropped ,the changes were statistically significant (P<0.05). The efficiency of normal behavior .of clinical treatment was 57.1%,and 44.4% of the control group.There were no differences in the treatment group and the control group (P>0.05). The two groups can improve life ability, normal habits, personality change score (P<0.05); The differences between the groups were not significant (P>0.05). There were more than 98.31% of the clinical group which achieved one class safety standard, 100% of the control group achieved. the differences between the two groups were not statistically significant.In short, Congzhi granules can improve the cognitive function state and the lower state of daily living of vascular dementia (P<0.05), the effict of Chongzhi granule is considered similar to hydergine's.The drugs can improve mild to moderate cognitive dysfunction in patients with vascular dementia and can improve the cognitive impairment caused by the drop in daiy and social life, so as to enhance the quality of life in patients. caregiver's burden was alleviated .At the same time, the pressure on society was reduced furtherly.
