Middle school student is a special group charactered by its biological and social meaning. What's the current state of their health? To what extent do they use health services? What's their potential health needs? These are focal questions concerned by the whole society.[Objects] To analyze the major health problems of middle school students, evaluate the state of their current health and find out their current and potential health needs. [Methods] Cluster random sampling was carried out in a field survey for 3352 middle school students coming from four grades of six different districts in Shandong province.[Results] The total annual prevalence rate of chronic and common diseases was 75.15%. mal-sight, rhinitis and pharyngitis, decayed tooth, anemia, malnutrition, neurasthenic and migraine, gastroenteritis, trachoma, abnormal bend of spondyle and bad dormancy were the leading ten diseases. The total occurring rate of injuries was 37.92%. Injuries in athletic sports, scald and burn by high temperature, bite and traffic accident were the leading four major injuries. The proportion of maladjustment was 3.3%, while the better was 49.2%, and between them was 37.6%. The total positive rate of symptom of mental diseases was 20% drew from the survey results. Constraint, susceptivity in relation of intercommunication, hostility, crankiness and dumps were the factors with higher positive rate. Lack of active attitude of medical treatment and hospitalization, ill-addiction and bad cognition, bad habits of bite and sup, bad sanitary habits and worse hygeian establishments, absence of physical exercise and deficiency of sleep, as well as inactive attitude to related education of health knowledge, were the outstanding health problems in risk behaviors and personal habits. The two-week prevalence rate was 134.3‰, among which the prevalence of respiratory system was highest. The disease of upper respiratory piping, tummy bug, rhinitis and pharyngitis, enteritis, acute bellyache and tonsillitis were the leading six diseases. The rate of seeking medical advice, in two weeks, was 93.08‰. The patients who did not seek medical advice accounted for 45.39‰ in all patients. The hospitalization rate was 17.6‰. The patients who should hospitalize but not accounted for 24.29‰.[Conclusion] There were many health problems among middle school students, such as common and chronic diseases, injuries, maladjustment, and syndrome of mental diseases, behavior-related risk factors and bad habits and so on. The exist of all these problems determined that current and potential health services were in great needs. The utilization of health services in middle school students was lower than the average level, no matter in-patient services or out-patient services. What's more, the health services, quantitively and qualitively, could not meet the actual needs of middle school students. The service ability was in short. In form and content, there were also big gaps in health service. And in service network, problems such as faultiness in service system, straitness in service area, weakness in ability of safeguard and deficiency in individual services and administrative levels were not improved. Health services for middle school students do need a change from cognition to action.
