AIMS: To discuss the etiology and appropriate methods of eye muscle surgery in highly myopic patients with esotropia and hypotropia. METHODS: 3 patients with unilateral or bilateral high myopia and strabismus were checked by MRI before surgery, one patient's recti extraocular muscles is normal, the other two with a pathological plane of lateral rectus found by MRI were confirmed intraoperatively. Recession of the medial rectus combination with resection of the lateral rectus, Yamada technology and orbital periosteal flap traction fixation surgery were performed. RESULTS: All eyes were corrected and ocular restrictive motility were improved. CONCLUSIONS: The etiology of highly myopic patients with esotropia and hypotropia is not clear, dislocation of the lateral mectus explains the etiology of some cases. Careful MRI examination before surgery will do help in analyzing the causes and making surgery Accurately. Recession of the medial rectus combination with resection of the lateral rectus is good for the early cases without dislocation of lateral rectus, Yamada technology is a perfect surgery for the cases with dislocation of lateral rectus, orbital periosteal flap tract fixation is suitable to the patients who have ocular restrictive motility disorder with limited abduction and elevation. Appropriate methods of eye muscle surgery in highly myopic patients with esotropia and hypotropia will still be under discussing.
