The Research For Syndrom And Syndrome Of The Diabetic Nephropathy Patients In The Pearl Triangle

Objective:Diabetic Nephropathy, which is the hackneyed capillary vessel syndrome, is one of the important reasons for death of diabetes. Till now, there is no effective remedy to prevent it from getting worse. There are still disadvantages such as nonstandard criterion of syndrome, incompetence to show the characteristic of Diabetic Nephropathy by syndrome and lack of epidemiology research . The purpose of this research is to provide external gist for syndrome criterion of Diabetic Nephropathy by investigating the syndrome of the Diabetic Nephropathy patients in the Pearl Triangle and the relationship among syndrome and stages of DN, Lab result. Method:Adopt the research methods of transect and retrospect. Choose the Diabetic Nephropathy in-patients of the main TCM hospitals in the Pearl Triangle , such as The First Hospital of Guangzhou TCM University , The Shenzhen Hospital of TCM, The Foshan Hospital of TCM, The Zhongshan Hospital of TCM, basing strictly on the choose criterion. Collect and analyze statistically the useful information, for example, the demography data, individual medical record, independent syndrome, laboratory examination results and so on. Result:This research comprises 292 cases, including 14 cases of the Ⅱ period, 42 cases of Ⅲ period and 201 cases of Ⅳ period. The most commonly seen independent symptom of Diabetic Nephropathy patients are lassitude, at 69.9%, and edema, at 57.2%,and energy sag at 54.1%,while always sigh, at 0.7%, and hectic fever and night sweating ,at 0.7%, are the least.Basing on the diagnose criterion of syndrome of deficiency of five viscera, syndrome of kidney deficiency is the main type and the rate is 37.7%, following by syndrome ofspleen deficiency, at 27.7%, whilst the syndrome of lung deficiency is the nonsignificant type. The II, III and IV period of Diabetic Nephropathy have the same leading syndrome —the syndrome of kidney deficiency, and the rates are 50%, 38.1%. 37.6% respectively. Basing on the criterion of deficiency of Yin-Yang and Qi-Blood, the percentage of main symptom is largest for the Yin deficiency and the next ones are Qi deficiency with the percentage of 19.5%, Yang deficiency with the percentage of 8.2% and Blood deficiency with the percentage of 9.6%.The rate of Yin deficiency is the highest type in not only the II, III but also the IV period of Diabetic Nephropathy, the rate is 35.7%, 19%, 34.8% respectively. Basing on the criterion of deficiency of syndrome of complicated deficiency, it is the syndrome of deficiency of both Qi and Yin that the most commonly seen syndrome(40.4%), following by Qi efficiency of both the spleen and lung, at 26.4%, whereas syndrome of deficiency of both Yin and Yang, at 3.8%, Qi efficiency of both the kidney and lung, at 4.8%, are the least. The percentage of syndrome of deficiency of both Qi and Yin is highest in the II, El and IV period of Diabetic Nephropathy, at 57.1%, 31%, 43.3% respectively. Basing on the criterion of demonstration, blood stagnation has the highest rate of 49.7%, the next one is syndrome of damp-heat with the rate of 33.2%,while Qi stagnation is seldom often. The percentage of syndrome of blood stagnation is highest in the II, III and IV period of Diabetic Nephropathy, at 50%, 50%, 49.5%, 50% respectively. There is no obvious difference between the syndrome of diabetes and the syndrome of Diabetic Nephropathy. Compared with the other syndrome of complicated deficiency, syndrome of both Yin and Yang has the highest rate of 24 hour proteinuira quantitative and the Yin deficiency of both liver and kidney has the lowest rate. Deficiency of spleen has the highest rate of Scr among the five viscera. The rate of Scr of Yang deficiency is highest among the syndrome of Yin- Yang-Qi-Blood while the one of Yin deficiency is the lowest. It is Qi deficiency of both the lung and kidney that highest among the syndrome of complicated deficiency, whereas the Yin deficiency of both liver and kidney is the least. In all demonstration, the percentage of Scr is highest in Syndrome of Water-damp whilst the percentage of Syndrome of damp-heat is the least. There is no distinct relationship among the types of DN, FIB and lipid metabolism. Conclusion:The results of the research of symptom and syndrome of Diabetic Nephropathy in the Pearl Triangle show that:1, There are differences between symptom of Diabetic Nephropathy and syndrome of Diabetic Nephropathy.2, The characteristics of Diabetic Nephropathy: Deficiency of kidney and deficiencyof spleen are the most ordinary syndromes in deficiency of five viscera, following by deficiency of heart, deficiency of liver and deficiency of lung;Yin deficiency and Qi deficiency are the commonly seen syndromes of Yin-Yang-Qi-Blood deficiency, the next one is blood deficiency and Yang deficiency;syndrome of deficiency of both Qi and Yin are commonly seen among the syndromes of complicated deficiency, and the next ones are Yang deficiency of spleen and kidney, Yin deficiency of liver and kidney, Qi deficiency of lung and kidney and syndrome of deficiency of both Yin and Yang;Blood stagnation and syndrome of damp-heat are general syndromes in syndrome of pathogens, following by syndrome of water-damp, Syndrome of damp-heat > and Qi stagnation.3 -. Diabetic Nephropathy is a disease of Biao excess and pathogens. If it is deficient, syndrome of deficiency of both Qi and Yin is most commonly seen and the morbid part is the spleen and kidney. If it is excess, blood stagnation is the important factor, but it is ordinary to see syndrome of water-damp in terminal Diabetic Nephropathy. It is syndrome of deficiency of both Qi and Yin and Kidney stagnation that the sticking points of the syndrome of Diabetic Nephropathy.
