Diabetic Nephropathy is a chronic, complicated and multi-symptoms disease, it is also known as Chronic Kidney Disease. A stage 3 CKD symptom is almost irreversible. Understanding the change of symptoms will help improving treating the disease; besides, it will also help researchers and physicians designing and redefining a most applicable treatment method. The Traditional Chinese Medicine (TCM) method of nourishing qi, invigorating blood, and tonifying yin has been used in clinic for more than 20 years. And it has been proven effective in improving the function of kidney. The purposes of this study are focusing on the long-term treatment effects of the above TCM method, the different treatment results in comparison to the western medicine, and the changes of the patterns of symptoms. Research Objectives:To compare the treatment effects between western medicine and the TCM method among stage 3 and stage 4 CKD patients. To observe how the TCM method affects the patterns of symptoms. Apply the informational entropy-based cluster method to analyze the change of patterns of symptom. To design and re-define the most applicable TCM formula. Research Method:To compare the treatment results by TCM formula and western medicine among CKD stage 3 and stage 4 patients by checking the urinary albumin, eGFR, GHB, CHO, TG, HDL, and LDL. To observe the patterns of symptom by using the informational entropy-based cluster method. Research Results:The TCM formula has a better treatment results than western medicine in urinary albumin, eGFR, GHB; CHO, TG, HDL and LDL. During the first follow-up, the conditions of 7 patients have been improve and moved from stage 3 up to stage 2 and the conditions of 11 patients become worse by progressing to stage 4. During the second follow-up, there are 15 stage 3 patients move up to stage 2 and the number of patients in stage 4 reduce to 9 patients. During the third follow-up,14 patients move up from stage 3 to stage 2 and the number of patient in stage 4 reduced to 5 patients. Comparing to control, there are four symptoms—heat in the palm and feet, palpitation, urination at night and frequent urination---have been greatly improved among patients in research group.The patterns of symptoms remain the same as heart-kidney yang deficiency plus qi and blood deficiency. There are three minor symptoms---qi blockage, fluid retention and severe internal heat---were observed.Conclusion:The composition of nourish qi, invigorate blood and nourish yin formula includes Pseudostellariae Radix, Radix Ophiopogonis, Angelica Sinensis, Rehmannia dride Rhizome, Macrocarpium Officinale, Poria Cocos, Alisma Orientale,,Radix Salviae, Mongolian Milkvetch Root, Eucommia Ulmoides Oliv., and its modifications has a better treatment effects than the western medicine.Comparing to the traditional method of identifying symptoms, the informational entropy-based cluster method is more effective in showing all symptoms and extracting the single symptom; it shows a clearer sequence of symptoms and provides with a better indications for a better TCM prescription.
