China is among the twenty-two high TB burden countries in the world, ranking the second in terms of total number of patients. Based on the investigation in 2000, there were 4.51 millions TB patients in China. The epidemic situation is more serious in corrections than the whole society. For long time TB has always been one of the main diseases affecting the health of prisoners. Based on the investigation report of the TB control work in corrections in China from September 2002 to March 2003, there were 19171 TB patients in all Chinese prisons and TB prevalence rate was 1251/10,00000,which is 3.4 times higher than that of common population.In recent years, more and more researchers begin to study the TB epidemic and use of health service in corrections. As for use of health service, each country has her own characteristic and there is no research on TB health service and related factors on accessibility in China at present. With the help of WHO (world health organization),we invested 255 tuberculosis patients from partly prisons of Shandong province and Liaoning province ,the main aim of this study is to describe the actuality of TB health service in prisons, to appraise the factors which affect TB health service ,to explore TB control technique fit for prisons and offer advice to related departments.Multi-case study was used in this study. The case is "prisons" with different TB epidemic situation and characteristic. Three prisons (one is medical prison, whose main job is providing health services for prisoners with diseases; the other two are common prisons) from Shandong province and one prison (medical prison) from Liaoning province were selected as the study sites in this study. Of those prisons, Weifang prison and Shandong Province prison are common prisons where TB prevalence rate was the most highest in Shandong province, Xinkang prison was the only medical prison in Shandong province, Tiding prison was the only medical prison in Liaoning province.
