Objective To explore the efficacy of fluoxetine in the treatment of depressive patients, influential factors of efficacy and the relationship between 5-HTT gene C/T polymorphism and clinical response of fluoxetine in the treatment of depressive patients. Methods In order to elucidate genetic and environmental influence on individualized response to antidepressant, from October 2003 to October 2005, a study on pharmacogenomics in depression was conducted in 14 hospitals in China. The current study was part of it. In total, 434 subjects were recruited to the current prospective study cohort. The efficacy and side effects of fluoxetine were assessed by HAMD, Zung, CGI and TESS before cure and at 1st, 2nd, 4th and 6th week of the treatment. 434 patients were genotyped for 5-HTT gene with Taqman technology. The influential factors of efficacy and the relationship between 5-HTT gene C/T polymorphism and clinical response of fluoxetine in the treatment of depressive patients were assessed with the multiple regression analysis. Results (1) The efficacy appeared at the 1st week. In the 6th week of the treatment, 73.1% patients showed response and 46.0% patients showed remission. There may have the correlation between age, HAMD baseline score, history of use of antidepressant, use of antianxiety drug and efficacy of fluoxetine. (2) There was no significant correlation between disease serious degree of depressive patients and 5-HTT gene C/T polymorphism. There was no significant correlation between efficacy of fluoxetine in the treatment of depressive patients and 5-HTT gene C/T polymorphism, however, a trend to improved efficacy of fluoxetine was demonstrated for TT patients in comparison to CC patients. There may have correlation between sexual dysfunction and 5-HTT gene C/T polymorphism.
