Objective: Rhegmatogenous retinal detachment(RRD)is one of the ocular diseases which cause blindness.Theattack rate of RRD is 0.179 percent.In anatomy,a majority of patien-ts' retina could be reattached through scleral buckling surgery.Thereattachment rate is up to 95%.But after retinal reattachment,macularcould occur abnormality of structure,of which is macular edema alm-ost.It could induce dysmorphopsia,ambiopia,et al.The orthodox ex-amining methods,for example,funds fluorescein angiography,presetlens of slit lamp,binocular inderect ophthalmoscope,et al,couldn'tcheck morphological changes of macuar exactly. OCT is a exact de-vice to examine macula uter.It's resolving power can reach 10μm ofretina.We examined macular structure of 45 patients with rhegmat-ogenous retinal detachment after scleral buckling surgery by OCT.This study is going to study the morphological changes of macularafter scleral buckling operation of RRD.Methods: We selected 45 patients(45 eyes)withrhegmatogenous retinal detachment .22 patients are men.23 patientsare women.The age is from 15 to 67 years old.The range of retinaldetachment is from 1 quadrant to 3 quadrants.All retinal breaks areon the peripheral retina.None of macular was involved in detachmentin all cases.We excluded cases which underwent any intraocularsurgery and cases with other intraocular diseases.Zeiss Stratus OCTModel 3000 (Stratus OCT) was used to study the morphologicalchanges of macular area on 45 patients (45eyes) before and aftersclera buckling operati-on.The feature of macular area was observedafter the operation in the 1st week, 3rd week, 5th week, 7th week,9th week, 11th week respectively.Results: In all 45 eyes, 18eyes(40%) had macularedema (ME). The thickness of neuroepithelial layer of macular areawas thinner gradually after operation, which was obvious in 5 wee-ks postoperation, and it needs a long time to recover the preoperationstate.Conclusion: The morphological changes of maculararea postoperation have strong association to the restoration of visualacuity.It is high proporation of macular edema after sclera buckling.The thickness of neuroepithelial layer of macular area was thinnergradually postoperation. With edema was subsided, visual acuity wasrecovered at the same time, But duration of macular edema was long,which was one of important factors to influent visual acuity reverse-bly. Visual acuity was elevated after operation. So in the terms ofpatients of RRD, an earlier, active treatment should be given, whichis a decisive factor to recover visual acuity.OCT can measure macular morphological structureaccurately and monitor the minimal changes, especially macularedema, which was proven by our research. So it can be utilized toobserve the morphological feature of macular edema after sclerabuckling operation of RRD.
