Objective: To investigate the characteristics and the influencing factors of cardiac structure and function in aged hypertension.Methods: 175 cases were selected from admitted patients from 2000 to 2004, 105 were over 80 years with hypertension(group A), including 80 men and 25 woman, 80-94 years old, average 82.21 years, the course of disease of hypertension is 0.5-50 years, every 22.09 years. 40 cases of 60-80 years old with hypertension(group B) and 30 cases over 80 years without hypertension(group C) were select as control.Among three group, sex , the ratio of coronary heart disease ,etc is matched, excluded secondary hypertension, diabetes and hypertrophic cardiomyopathy. 105 cases were divided into <10 years, 10-20 years,20-30 years,> 30 years according to the course of disease;divided into light intermediate hypertension and severe hypertension according to the hypertension level. Color Doppler echocardiogram were observed in all patients and compared.Result:Group A, compared with group C, LVM , LVMI,, LVD increase significantly, EF, FS decreased significantly (P<0.01), IVSD , LVPWD , LAD obviously increase, E/A obviously decreased (P<0.05 );compared with group B ,LVMI increased, there is statistics meaning (P<0.05 ), EF , FS , E/A reduced (P<0.01 ) significantly. After correcting hypertension course of disease , weight index and influence of the area of body surface, the difference of LVMI disappears . Among the inferior group of different course of hypertension disease, LVM , LVMI , IVSD , EF, FS demonstrate the prominent statistics difference (P<0.01 ), with the extension of the course of disease of hyperten -sion, LVM , LVMI , IVSD increase significantly, EF , FS obviously reduce, LAD and E/A although increase and lower, the difference is not obvious. Increase by level with blood pressure, LVM , LVMI, LVD , EF, FS demonstrate the significant statistics difference (P<0.01 ), IVSD , LAD demonstrate the statistics difference (P<0.05 ). There were no statistics difference in every index within severe hypertension and light intermediate hypertension two inferior group,Conclusion: Increased age and hypertension made cardial enlarged, leftventricular hypertrophy and systolic function decreased,;With the extension of the course of disease of hypertension and rising of blood pressure level, the changes of cardial structure and function aggravate gradually;It is not the same to increase age and impact on heart of hypertension: Hypertension cause left ventricular hypertrophy and systolic function fail , increased age impact on relaxation function mainly.
