Objective: To investigate the change of family dynamics in orthopedic injury patient's family and explore the family nursing diagnosis and interventions in orthopedics department.Method: The study was conducted in three parts. In the first part, Family Dynamics Measure was introduced to China, and its reliability and validity were evaluated. In the following part, the change of family dynamics in orthopedic injury patient's family and its predictors were evaluated by Family Dynamics Measure. In the last part, the cause and manifestation of change of family dynamics in orthopedic injury patient's family were analyzed by doing qualitative research.Result: In general, family dynamics in orthopedic injury patient's family were decreased in early stage. The patients and family members reported more enmeshment, unclear communication, role conflict and more mutuality. The change of family dynamics in orthopedic injury patient's family were effected by age and carrier difficulties, and the decreased patients compliance were related to pain, worrying about the wound's healing up, absence of right understanding to exercises and absence of supervision or direction.Conclusion: The decrease of family dynamics in orthopedic injury patient's family effected the rehabilitation of the patients. Clinical nurses should direct the family in building up individuality, learning more communication skills and adapting to role changes to improve family supports to the patient's rehabilitation.
