Sarcomatoid carcinoma is a rare biphasic tumor characterized by a combination of malignant epithelial and mesenchymal cells. Very few cases have been observed in the gastrointestinal tract. To our knowledge only two cases with rectal involvement have been published to date. We report a case of sarcomatoid carcinoma of the rectum in a 60-Year-old man hospitalized with a history of bloody stools. The patient underwent a abdominoperineal resection under the impression of colonic carcinoma.. Histologically, the tumor was composed of undifferentiated carcinomatous and sarcomatous components. Immunohistochemically, tumor cells showed a positive immunoreaction for both cytokeratin and vimentin. Widespread metastases in the liver were found 2 months after the operation. Our review of the literature stresses the poor prognosis associated with rectal sarcomatoid carcinoma.
