PrefaceWith the development of shadowdiagnosis technique, the detective rate of cholangiocarcinoma increases. Because cholangiocarcinoma may metastasis via lymphatic, bile duct and nerver at early stage, infiltrating growth is its major feature of pathology. Most patients have not special clinical situation and most patients are late stage at first presentation, its local infilitration and remote metastasis lead to difficult radical operation. Survival rate is low and prognosis is poor after operation. With the development of the method of molecularbiotics and its application in pathohistology, genes are noticed more by learners in the area of tumor study. As a later found inhibitor apoptosis protein (IAP) , surviving is researched by most people. Survivin appears during the development process of embryo. It is not expressed in adult determinate cell, and it is expressed obviously in nearly all the human tumor cells (including melanoma, neuroblastoma, breast cancer, lung cancer, gastrointestinal, urogenital, blood system carcinoma).In this study, we will examine the expression of surviving in cholangiocarcinoma by immunohistochemistry ( S - P). And by intergrating clinical data, we will verify the expression of survivin, and the relationship with clinic pathological significance of cholangiocarcinoma, and the relationship with another anti-cancer gene - P53. We will analyze the conclusion in order to supply theoric bases for diagnose and comprehensive treatment.Methods1. Tissue specimens collection;41 cholangiocarcinoma specimens as exper-iment from 1999 to 2005 in the First Affiliated Hospital Of China Medical University were obtained. 12 normal bile duct tissues as control, which is from transplant samples. All the samples were confirmed by H&E before experiment.2. Main agents: survivin antibody (mouse monoclonal antibody for human) is from Neomarkers company in USA. P53 antibody (mouse monoclonal antibod-y for human) is from Santa Cruz company in USA. S - P agents and DAB agents are from zhongsan Biological company.3. Methods;The 41 cholangiocarcinoma specimens and 12 normal bile duct tissues were stained by immunohistochemistry(S — P) and the expression of surviving and P53 was clarified with microscope. Chi square test was used with SPSS12. 0 sofe ware.Results1. The expression of survivin in cholangiocarcinoma was higher than that in normal bile duct tissues, there was significant difference between them (P <0. 01).2. There was no significant difference in the expression of survivin in difference sex N age ^ clinical stage^pathotyping^lympy metastasis. (P>0.05).3. The expression of survivin in different P53 expression was significant difference. There was significant difference among them. (P<0.05).DiscussionInhibitor of apoptosis protein (IAP) was isolated by Crook in 1993. Survivin is one of the most important IAP, which was found by Ambrosini in 1991. It appears during the development process of embro, which is not express in a-dult cell (not including thymus) ,and it is expressed obviously in nearly all the human tumor cell ( such as melanoma, neuroblastoma, breast cancer, lung cancer , gastrointestinal cancer, etc).Its mechamism is :1. To inhibit the active of caspase 3 and 7 directly.2. To express specific in one of cell cycle------G2 - M. Survivin is integratewith microbule. This modulates the 3. transcribe of cell gene and inhibit the cell apoptosis.3. To facilitate angiogenesis.P53 is one of suppression - oncogene, which plays a role as anti - cancer by modulating the growth and apoptosis of cells.The experiment indicates that survivin is overexpressed in cholantiocarcino-ma. It may play an important role in the carinogene of cholantiocarcinoma. Inactive of antioncogene P53 and expression of survivin may play synergetic role in carcinogenesis of cholantiocarcinoma.Conclusion1. There was a high level of survivin in cholangiocarcinoma.2. There was no relationship between the expression of survivin and sex^ age Nclinical stage ^pathotyping^lympy metastasis.3. There was a significant relationship between the expression of survivin and P53. This can supply theoric bases for diagnose and comprehensive treatment.
