Objective To study the clinical feature, treatment and prognosis of parathyroid carcinoma. Methods Clinical and pathological data are analyzed retrospectively on 30 cases from 1978 to 2003. Results The results show that the ratio of male to female is 1:1.3 and the median age is 39. The most clinical feature is skeleton pain and muscle atony (50.0%,14/28),next is mass in the neck (14.3%. 4/28) and both of them(7.1%,2/28).The other feature includes gastroenteric symptom like naupathia and vomit(7.1%2/28),hematuria (3.57%,l/28)and neurasthenic(3.57%,l/28). And there are 4 cases are diagnosed as functional parathyroid adenoma at first and the tumor recur repetitiously after the first operation then to be are diagnosed as parathyroid carcinoma(14.3%,4/28).90% of them have hyperparathyroidism.Renal and skeletal involvement respectively occurs in 32.1%(9/28) and 82.1%(23/28) .And 28.6%(8/28) of patients have both of them. 24 cases show the method of operation:13 of them are simple parathyroidectomy,2 of them are en bloc resection(thyroid gland plus neck lymph nodes) ,6 of them are simplex resection of the tumour,and 1 of them are partial resection of the tumour. 17 cases report the prognosis, 1 case does not show the time of recrudescence, 5 of them recur respectively 4,6, 13, 18 and 36 months after the first operation and among the 5cases,two are dead because of local recrudescence 23 and 35 months respectively after the first operation. and the other 10 cases do not recur after median 12 months' observation.Conclusion Enhancing the consciousness of parathyroid carcinoma., perfect examination before operation, detecting the level of serum calcium and PTH and associated image examination is useful to conform the diagnose of parathyroid carcinoma. It is very important to resect the tumor with the ipsilateralthymoid lobe as complete as possible at the first time.
