Emergency treatment is a new and frontier comprehensive science of medicine, which involves almost every aspects of medicine. Now, as main component of emergency treatment medical system and key functional organization for development of emergency treatment medicine, the emergency treatment in a hospital is playing more and more important and active role in medical field. Through investigation into actual situations in many hospitals, the author of this thesis analyzes the present situation of emergency treatment in hospitals, and forwards corresponding countermeasures for development, which provides basis for establishment of emergency treatment management mode for hospitals in our country. Emergency treatment is a clinic medicine to study patients'diagnosis and treatment, and it is an independent clinic medicine developed in the past 30 years. The medicine of emergency treatment refers to the normal medical theory and skills that medical worker can use to carry out first aid to patients in need of emergency treatment, and restore the life symbols of patients within the shortest duration. There are both difference and relations. Emergency treatment is featured by time limit, unpredictable nature, complexity, and coordination and condition reliance. Mission of emergency treatment section is to receive the patients, and give first aid to patients suffering from serious diseases. The emergency treatment section is an important department to meet emergent public health concerns and the kernel of emergency treatment medical service system. Now, facing frequent occurrence of highly dangerous industries in our country, to do emergency treatment better shall be of special significance. Most hospitals in most area of our country have established their emergency treatment sections, and some hospitals have even set up their ICU. In general, the hardware grows fast, but the software cannot catch up with it. Also due to lack in professional workers, more than 20 years'development fails to constitute perfect and meaningful specialty of emergency treatment in our country, and personnel configuration of emergency treatment section is not centralized, and the foundation of education for emergency treatment in our country is very weak and backward. The author of this thesis has collected and processed data and information about the emergency treatment system of hospitals in the United States, Japan and France. Compared with the developed countries, the emergency treatment of hospitals in our country is respectively backward. To understand present situation of emergency treatment of hospitals in our city, this thesis aims at 6 hospitals located within the city proper of Changchun to carry out the investigation into hospitals'scale, resources, instrument, personnel configuration, emergency treatment process flow, emergent prevention measures, disciplinary construction, continuous education, environment service, case management and medical disputes, etc. In the 6 hospitals investigated, there are 3 large-scale hospitals, receiving 2300 outpatients or so each day; and 3 medium sized hospitals, receiving 680 outpatients or so each day. As for organizational form, normally, the large-scale hospitals adopt semi-independent mode, while for the medium sized hospitals, one adopts the semi-independent mode and two adopt the passage mode. In all hospitals investigated, only one can provide before-hospitalization rescue, while the other five have not. All the hospitals investigated are equipped with respirator, quiver eliminator, ECG monitor, stomach washer, and phlegm extractor. However, thedifference lies in the fact that hospitals of different scale have different quantities and models of above equipment. 60% of the hospitals investigated assign special persons to maintain the equipment and instruments; and normally, the emergency treatment section has the first-aid room, disposal room, observation room, surgical treatment room, emergency treatment clinic. The large-scale hospitals even have operating room and emergency treatment zone, etc. Most emergency sections of comprehensive hospitals in Changchun City use the rotary organizational structure, but nurses are 100% fixed. Large-scale hospitals have 16-30 nurses, and medium sized hospitals have 7-13. Emergency treatment doctors and nurses have higher length of working years, and higher educational background, however, average age of doctors and nurses working in medium sized hospitals is higher that of those working in large-scale hospitals, and so is the academic titles. 83.3% of the X-ray section, electronic diagnosis section, and inspection section are located near the emergency treatment section to facilitate rescue and inspection of the patients seeking for emergency treatment. Patients suffering from serious diseases can use the green passage for first aid, which saves the time of treatment, and embodies the principle of the hospitals to take the patients as the center, and improves the success rate of emergency treatment. Most doctors and nurses in emergency treatment sections are warmhearted to patients, who can communicate with patients and their families in a good way. All the emergency treatment sections carry out the initial treatment responsibility system, 3-level doctors responsibility system, and emergency treatment consultation system, etc. Normally, medical record for emergency treatment is written in good order like that for inpatients. Concerning medical record for serious patients, the doctors and nurses should also keep the first aidrecord. Teaching hospitals have already become independent discipline for academic construction, and they have the development orientation. Some of the emergency treatment sections lack emergent plan for major emergency treatment and abrupt events. 50% of the hospitals have personnel training plan while the other 50% have not. 100% of the hospitals have experienced violent events for emergency treatment, and more medical complains, and 50% of the doctors and nurses think that the leaders pay insufficient care. Main problems existing in emergency treatment in our city are imperfect management system, unclear working functions; poor emergency treatment equipment, backward functions; poorly configured personnel, unstable emergency treatment team; and weak surgery force for emergency treatment. Factors affecting emergency treatment of the hospitals include policy, environment and other external conditions, management system of hospitals, degree of leaders'attention, quality of first-aid workers, medical equipment and instruments, relations with other clinic sections, and conflicts between patients demand for psychological service and service provided, etc. Countermeasures for improving emergency treatment management work of hospitals: Firstly, scientific emergency treatment pattern should be established, and management system of emergency treatment should be improved, including: establishing service concept; strengthening leadership by setting up command system of emergency treatment; preparing counter plan against emergency to improve reacting capacity; formulating related regulations to guarantee medical service quality; assigning personnel reasonably to make full use of manpower resources; providing regular technical training to better quality of professionals. Secondly, attention must be paid to environment transformation, and matching facilities shall be perfected. Thirdly, construction of emergency treatment information systemmust be strengthened, and working process flow should be simplified. Fourthly, attention should be paid to medical security, and medical disputes should be avoided. Fifthly, quality assessment standard and system of emergency treatment should be constituted, and quality supervision and management should be carried out. Sixthly, disciplinary construction and scientific research of emergency treatment should be accelerated. In general, the decisive factor for development of emergency treatment section of hospitals should be scientific management, i.e. to carry out configuration and effective operation of various elements such as manpower, finance, material, time, and information, etc., to fully exert function of various resources, and to realized the maximized social service for the public. Po provide excellent service to the public under the limited medical conditions, the only way to go is to strengthen internal management, obtain excellent quality from management, and obtain efficiency from management. In such a way, we can satisfy patients with excellent service, and promote the development of emergency treatment medicine uninterruptedly.
