Objective The investigation was to evulate the efficiency of the frozen shoulder treated by manipulation with acupotomy in tensive position and to approach it's theory and characteristic.Methods Between May 2003 and January 2005, 66 patients with frozen shoulders, aged from 40 to 70 years, were enrolled the controlled study, who were divided into two groups randomly. 40 patients (including 17 male and 23 female cases) were treated by manipulation with acupotomy in tensive position (Group A) and 26 patients (including 13 male and 13 female cases) were treated by manipulation maneuver (Group B). Both group received the same rehabilitation exercise after therapy. The motion of the shoulder and the pain degree were observed during the treatment and the follow time was 3~12 months.Results AH the patients were followed up 3-12 months (average 7.59 months). The percentage of excellent and good results in Group A in 3 month and 6 month after the treatment was 95.00% and 96.97% respectively, while that of Group B was 46.15% and 60.00%. Group A showed significant advantages compared to Group B, and complicatioms were not found in both groups.Conclusions The characteristic of the treatment by manipulation with acupotomy in tensive position is manipulating the adhesions and contracturesof capsule in direction: including the zona of intertubercular sulcus, inferior-acromial and the decocted later part of the capsule, and with hydraulic distension and manipulation. Compared with manipulation maneuver this treatment is less traumatic, short of course of treatment, easy to perform and really an effective procedure to treat the frozen shoulder.
