Qualitative research method was applied in this study. A survey was done by conducting interviews with 12 head nurses of 7 non-profitable 3-A-Class hospitals in ChengDu who were considered as having better leadership effectiveness by their nursing departments. By the interview, the various supporting factors and barriers which influence the achievement of effective management and leadership effectiveness in their characteristic and external environment were deeply understood. Grounded theory was applied to collect, analyze and arrange interview data. The relationship between the achievement of leadership effectiveness and the various factors in the internal and external environment was also deeply explored. The profound significance and practice of the achievement of leadership effectiveness of the nurse leaders were then discussed. Based on these we build an interpretive theory on leadership effectiveness of nurse manager. The research results are as following :Factors from the nurse manager herself in the internal environment such as good individual character > fine specialized quality and good leadership ability etc are beneficial for the achievement of leadership effectiveness. Good individual characters mainly include suitable values, right way of thinking and good behavioral performance. Fine specialized quality is reflected in the steady professional belief, profound professional kowledege and the practical work competence. Good leadership ability is mainly embodied in dealing with interpersonal relationship , communicating in the organization , discerning and empowering ,taking the whole situation into account and planning accordingly, reforming and innovating ,making and implementing correct decision.The factors from the external enviorment such as society , hospital organization , nursing profession and nursing staff effect the realization of the leadership effectiveness. The supporting factors include the well awareness of health-care of the people in the whole society , the fine marketing and managerial mechanism of the hospital organization, the bright future of nursing professional development, the installation various occupational post ,the higher professional technological value ,active motivation of self-realization of nurses, the well ability of self-realization, the higher self-value criteria and etc. The obstructive factors include the superficial conception of conventional social value on nursing profession, the more responsibility of female posed by the traditional custom of the society , the conservative managerial environment, the unreasonable managerial organization and occupational function installation, the imperfect utility of the hospital staff, the little attention on the development of nursing professionand the lack recognition of individual development, the relative lower starting point of nursing profession and the long process of improving the comprehensive quality of nursing staff.The relation between the achievement of nurse leader's leadership effectiveness and the various relative factors in the internal and external enviorment embodies in the interaction of the factors in the internal and external enviorment. The achievement of the leadership effectiveness equal to the characters of the nurse leaders and the realization of self-value and the professional value of constant development of nursing profession. The managerial mechanism and atmosphere of hospital organization effect the realization of leadership effectiveness directly. The well awareness of health-care of the people in the whole society and the traditional conception and custom effect the realization of leadership effectiveness indirectly.The profound significance and practice of the achievement of leadership effectiveness reflect in the achievement and promotion of the value of nurses , nurse leaders and the professional value of the development of nursing and the value of the hospital station in the society, and embody in the offering of the higher quality nursing service according the increasing healthy requirement of people in the whole society and in the creation of better social and economical interest.
