Objective: To compare the clinical effect on the unstable intertrochanteric fractures in the eldly between the prosthetic replacement and the internal fixation and to approach the indication, methods and announcement and treatment before operation, during option and after option and. Furthermore, to investigate the viability of the freely control study in large scale between these two treatments. Documents and Methods: The 15 patients dealt with prosthetic replacement were selected from January 1988 to February 2005 in the China-Japan Union hospital of Jinlin Universicity. The patients'ages were all older than 60 and belonged to the unstable types according to Evans-Jensen classification. The flup was between 3 months to 3 years, and the average time of flow-up visit was15 months. While the 26 advanced age patients suffered by the unstable intertrochanteric fractures in the eldly in the internal fixation group were treated with sliding screw devices and these patients were chosen from January 1996 to December 2002. The ages were between 62 to 84 and the average age was 74.1. The flup time was between 2 years to 4years. Recorded the time and blood-transfusion during operation in both groups, and fluped the ambulatory time, the occurrence of complications.
