Objective To study the relationship of Survivin and Caspase-3 protein expression with Helicobacter pylori infection in gastric carcinoma and its association with clinical pathological significance. Mothods The Helicobacter pylori infection was detected by urease test and Warthin - Starry staining. The expression of Survivin and Caspase-3 was investigated immunohistochemically in 60 gastric cancer patients without the history of radioactive or chemical therapy. We studied their relationship with the clinicopathologic parameters of gastric cancer. Results (1), No Helicobacter pylori was discovered in tissue of gastric cancer; The Helicobacter pylori infection was detected in the surrounding tissue of gastric cancer in 38 of 60 patients. (2), The overall positive rate of Survivn in 60 cases with gastric cancer is 75% , No Survivin was found in their neighboring tissue in 60 cases . While overall positive rate of Caspase-3 in gastric cancer tissue was 85%, but 93% in their neighboring tissue in 60 cases with gastric cancer. (3), The positive rate of Survivin , Caspase - 3 has no relation to gender andage , while they show positive and negative relation to Helicobacter pylori infection separately. ( 4 ), The expression of Survivin was significantly associated with different histological grades, The expression rate of Survivin was higher in later clinical pathological stages of gastric cancer. The expression rate of Caspase - 3 was lower in low histological grades, but no relationship with clinical pathological stages. The expression of Survivin in carcinoma was not directly related to Caspase -3. Conclusions In this study, we found that Survivin issignificantly increased in gastric cancer tissue, in contrast , it wasn't detected in their neighboring tissue. On the other hand, the expression of Caspase - 3 is higher in their neighboring tissue than in gastric cancer tissue. Furthermore Survivin has positive relation to Helicobacter pylori infection, but Caspase - 3 has negative relation to it. Our results suggest that Helicobacter pylori infection may upregulates the expression of Survivin gene and downregulate the expression of Caspase - 3 gene in gastric cancer tissue, which inhibits the apoptosis of gastric cancer. This might be one of the mechanisms of H. pylori inducing gastric cancer.
