Objective:To investigate the association of expression of heat shock protein 70, trefoil factor1 in gastric cancer and precancerous diseases with H. pylori infection and the clinical significance.Methods:86 cases with endoscopic performed, including 32 with gastric cancer,26 with chronic atrophic gastritis and 28 with gastric ulcer, were carried out. Hp were tested in 86 cases by rapid urease test and warthin-Starry stain, accourding to results tested, three kinds of cases were divided into Hp+ group and Hp- group. the expression of TFF1 and HSP70 were tested in each group by immunohistochemistry. The association of the expression HSP70 and TFF1 with Hp infection was analysed.Results:1.HSP70 in gastric ulcer, chronic atrophic gastritis and gastric cancer was gradually increased. In the three kind of diseases, the expression of HSP70 in Hp+ group was higher than Hp-group (optical density values were:gastric ulcer Hp-group 0.289±0.012, Hp+ group 0.302±0.011; chronic atrophic gastritis Hp-group 0.304±0.010, Hp+ group 0.317±0.013; gastric cancer Hp-group 0.319±0.015, Hp+ group 0.335±0.018), there was significant difference(P<0.05).2.Compared Hp+ group with Hp-group, expression of TFF1 in gastric cancer did not have significant difference (P> 0.05); (optical density values of Hp+ group were 0.331±0.019, Hp-group 0.335±0.020), comparied gastric ulcers and chronic atrophic gastritis,the expression of TFF1 in gastric cancer was significantly low. The express of TFF1 in the gastric ulcer and chronic atrophic gastritis, Hp+ group was significantly higher than the Hp-group,with significant difference (P<0.05). (optical density values are:chronic atrophic gastritis Hp-group 0.368±0.025, Hp+ group 0.385±0.022; ulcer Hp group 0.346±0.030, Hp+ group 0.362±0.023).Conclusion:1.In the mucosa of gastric ulcer, chronic atrophic gastritis and gastric cancer, HSP70 expression gradually increased, associating with Hp infection significantly, that is, the expression of Hp+ group was significantly higher than Hp group.2.TFF1 expression in gastric cancer was lower than gastric ulcer and chronic atrophic gastritis, having no association with Hp infection, but TFF1 expression in gastric ulcer and chronic atrophic gastritis was associated with Hp infection significantly, that is, the expression of Hp+ group was significantly higher than Hp group.3.HSP70 and TFF1 may be an important protective factor of gastric mucosa, through a variety of ways to play a protective effect on gastric mucosa.
