Objective: To summarize the clinical features and surgical management of primary retroperitoneal malignant tumors(PRMT) and explore the best surgical treatment.Methods: Retrospectively analyzed 22 patients of PRMT in our hospital between December 1999 and September 2004, including the clinical features, surgical managetment, postoperative recurrence and prognosis.Results: The most common clinical manifestations in 22 patients of PRMT were abdominal mass(50.0%) and abdominal pain(40.9%). Histological types of PRMT were complicated, and the most usual one was liposarcoma(45.5%). B ultrasonography and CT were familiar assistant examination methods. There were 21 patients who underwent attempted surgical resections. 13 of the 21 patients underwent complete resection. The rate of complete resection was 61.9%. 6patients who underwent complete resections developed 9 locoregional recurrence all together. The rate of locoregional recurrence was 46.2%. And 5 of them underwent 7 re-resections all together. The rate of re-resection was 77.8%. 19 patients were followed up for 1~72 months (averaged 28.7 + 22.8 months). The survival rates of 1, 3, 5 year of the whole surgical group were 76.0%, 76.0%, 50% respectively. The survival rates of 1, 3, 5 year of the complete resection group were 91.6%,91.6%, 68.8% respectively. And the survival rates of 1, 3, 5 year of the non-complete resection group were 50%, 50%, 33.3%respectively.Conclusion: The clinical manifestation, histological type and surgical treatment are complex in PRMT. The survival rate of the complete resection group was higher than the non-complete resection group (P<0.05). Through raising the percentage of complete resection and carrying out operation again on the recurrings, it is obvious to find that patients can have longer life time.
