Objective To determine the role of Vascular endothelial growth factor (VEGF) and Erythropoietin(EPO) in the pathogenesis of endometriosis and to evaluate their diagnostic values. Methods The levels of VEGF and EPO in PF(peritoneal fluid) and serum were measured using enzyme linked immunosorbent assay(ELISA) from women with endometriosis(n=44 stageI-II 10,stageIII 20,stage IV 14) and without endometriosis((control group n=20)and in 3 months after treatment with Progestin(counterback group n=18). To analyze the relation between the level of VEGF and EPO in PF with endometriosis and r-AFS and compare the concentration of VEGF and EPO in PF and serum. At the same time, receiver operator characteristic curve(ROC)was used to analyze the sensitivity and specificity of VEGF and EPO in PF in the diagnois of EM. Result (1) The PF levels of VEGF and EPO with endometriosis were higher than that in control group(P<0.05). Furthmore, EPO concentrations in PF from patients with stage I-II disease were significantly higher than those with III,IV disease (P<0.05); (2) Significant negative correlation was found between EPO concentration in PF and r-AFS score(r=-0.625,P<0.01); (3) The VEGF and EPO levels in patient with endometriosis in PF were significantly higher than those in serum. (P<0.05); (4) The serum levels of VEGF and EPO in patient with endometriosis were significantly higher than those in the control group and the counterback group (P<0.05); (5) There were not obvious correlation between the PF levels of VEGF and EPO in patient with endometriosis(r=-0.193,P>0.05). There were not obvious correlation between the serum levels of VEGF and EPO in patient with endometriosis(r=-0.220,P>0.05). Neither the levels of VEGF nor EPO was obvious correlation in serum and PF in patient with endometriosis(P>0.05); (6) VEGF in PF and serum and EPO in PF will be helpful for the diagnosis of EM.Conclusion These data suggest that VEGF,EPO may play a important role in the pathogenesis of endometriosis, especially EPO in the initiation of the disease. Antiangiogenenic agents may provide a novel therapeutic approach for the treatment of andometriosis.
