Objectives:Esophageal carcinoma has a high morbidityin our country.Patients with this disease have a very low five years survival rate and the metastasis is very quickly. This disease is harmful to health.In recent years,although surgery technique,chemotherapy and radiotherapy have a soquickly development, the survival rate has not risen paral- lelly.The reason is micrometastasis through the lymph or blood vessels before the surgery.Investigation found that some of the carcinoma could restrain the growth of metastatic focus,no metastatic carci- noma would grow quickly only after the original carcinomawas ablated.Researchs of these reasons made many blood vessel growth depressor found.Endostatin is one of them, which was found by O'Reilly in Harvard 1997.It was dis-tilled from Eoma culture medium first. It is a proteolytic fragment of collagen type XVⅢ,which is a proten inhabitor of angiogenesis.Its molecular weight is 20KD.Many academicians have had many studies on it recently.It is lcocated at tissue fundus,mostly in some of highly blood vessel organs. Endostatin restrains the fashion of new blood vessel,including chicken embryo allantoid velum,big rat cornea,artery annulus culture medium,and so on.Endostatin can restrain the transference of inner cell and the multiplication of blood vessel endotheliocyte cell stimulated by bFGF and so on at cell levels.Many academicians carried through a lot of studies on the characteristics of endostatin.And they proved that endostatin had close relations with many carcinomas' growth ,metastasis and prognosis,such as lung cancer ,liver cancer ,kidney cancer and so on.It can reduce the blood vessel numbers of carcinoma,increase the wither of carcinoma cell.So that it can resist carcinoma. Endostatin has been in clinical research in America.We have not found the related study on expression ofendostatin in esophageal carcinoma by now. We wanted toanalyse the influence of endostatin to esophageal carcinomalong-dated survival and the relationship between expressionof endostatin and clinical and physiological characteristics in the esophageal carcinoma through examining the levels of endostatin in the tissue of esophageal carcinoma ,in thatof nub and in that of benign esophageal disease.Moreover, to evaluate the synthesis therapy value in the application of endostatin to esophageal squamous cell carcinoma.Method:We analysed the endostatin levels of esopha- geal carcinoma and in the nub of esophageal carcinoma byapplying immunohistochemical staining(SP method).We tookthe tissue of esophagus from the benign esophageal diseasepatients in the corresponding period for comparison. The samples were all from the esophageal disease patients the- rapied in the Department of Thoracic Surgery of the ForthHospital of HeBei Medical University from January 1996 to December 1996,of which 62 cases were esophageal car-cinoma,14 were benign esophageal disease patients.All ca- ses were not under chemotherapy and radiotherapy.Results:(1)The expression of endostatin was located atthe carcinoma cells,the esophageal mucosa,the vascular end-othelial cells,plasm cells,and part of lymphocyte cytoplasm.(2)Comparison of the expression of endostatin in different esophageal tisssues:the total positive expression in esopha- gus carcinoma was 73.13%±7.62%,which was higher thanthat in the tissues of nub and benign esophageal disease, 50.14%±5.51%and 40.36%±8.56%respectively(p<0.01),andpositive expression of endostatin in nub was higher than benign esophageal disease(p<0.01).(3)Relationship between the expression of endostatin and clinical and pathophysiolo-gical characteristics in esophageal carcinoma tissue:the exp-ression of endostatin in esophageal carcinoma was closelyrelated to the invasion degree of primary tumor,distant me-tastasis of cancer,clinical TNM grades and cell differentia- tion of esophageal carcinoma (p<0.01),but not to locations of the tumor,area lymph nodes metastasis,age,sex,habits ofeating and history of family(p>0.05).
