Objectives: The first Objectives of the study is to research the relation between occurrence of simple bone cyst and absorption of bone matrix and likely pathogenesis of simple bone cyst. The second Objectives of it is to seek the markers that can reflect activity of simple bone cyst. The third Objectives of it is to investigate the relation between bone alkaline phosphatase and osteocalcin. Methods: 13 cases with middle or little simple bone cyst in which have 10 males and 3 females between 4 years 3 months and 11 years 10 months old (mean age was 8 years 11 months old) were treated by surgery. The cyst was located in the humerus in 5 patients, in the femur in 7,and in the tibia in 1. The pure cyst fluid was aspirated at the same time of surgery and then chemical examinated. 5 cases with large simple bone cyst in which have 4 males and 1 females between 4 years 1 months and 10 years 7 months old (mean age was 8 years 2 months old) were treated by the needle aspiration of cyst fluid and acetic acid methylprednisolone injected into the cavity. The cyst was located in the humerus in 1 patient, in the femur in 3, and in the tibia in 1. The pure cyst fluid was chemical examinated too. The patients with large simple bone cyst were regularly followed up, and were treated by surgery when the cavity had becamed little enough to achieve the standard of middle or little simple bone cyst after 1—4 times aspiration of cyst fluid and hormones injection. The cyst fluid of large simple bone cyst was aspirated 12 times in all. The pure cyst fluid was also aspirated at the same time of surgery and then chemical examinated which have 18 times in all. The venous blood in same limb of the patients with simple bone cyst was collected at the same time of aspiration of cyst fluid and then chemical examinated and 18 results were random selected to bring into the investigation. 18 patients with transient synovitis of the hip , atlantoaxial rotary displacement or gluteus contracture et al which voluntarily accepted examination and hadn't bone disease were selected to compare with. The check marker in all sample was calcium, phosphorus, 25(OH)D3, ALP, BALP and BGP.Results: 1. The content of calcium, phosphorus, ALP, BALP and BGP in cyst fluid with large simple bone cyst were much higher than that in cyst fluid with little or middle simple bone cyst and in blood serum of the patients; and the content of 25(OH)D3 in the former were much lower than that in the latter. For each of the six parameters were found between all groups had statistics significances. 2. The content of calcium, phosphorus, ALP, BALP and BGP in cyst fluid with little or middle simple bone cyst were much higher than that in blood serum of the patients; and the content of 25(OH)D3 in the former were much lower than that in the latter. For each of the six parameters were found between the two groups had statistics significances.3. The content of calcium, phosphorus, 25(OH)D3, ALP, BALP and BGP in the blood serum between patients and normal contrast groups had no significant differiences.4. There was a significant between measurement of BALP and BGP in all samples and the correlation coefficient was 0.841. Conclusions:1. The content of calcium, phosphorus, ALP, BALP and BGP in cyst fluid could be took as important markers which reflect activity of simple bone cyst. More higher of the content of calcium, phosphorus, in cyst fluid and more lower of the content of 25(OH)D3, the activity of simple bone cyst was more higher.2. The relation between BALP and BGP was correlatedly and they had better correlation. Select anyone of the two measurements, the bone matrix metabolism would be better reflected.3. The etiology of simple bone cyst was caused not only by venrous disturbances but also by bone matrix absorption, but was the results caused by many reasons.
