The Influences Of The Health Reform "Patient's Selection For The Doctor" On Service Behaviors Of The Medical Workers In Shaoxing Hospitals

PurposeTo understand the influences of " patient's selection for the doctor " on medical worker's job behavior; To analyse and compare the difference of all kind of medical workers and patient judgement on influence of "patient's selection for the doctor " to medical worker job behavior. To explore the rational suggestion for medical organization to promote and perfect the reform of " patient's selection for the doctor". MethodWe collect 915 medical workers and 300 impatients from two general hospitals and section hospitals which are carrying out the reform of" patient's selection for the doctor " program in shaoxing city.We studay and analyse the influences of " patient's selection for the doctor " on medical worker's medical ethics and atmosphere, medical worker's group cooperating, medical quality, medical worker's economic behaviour and medical worker's science and education behavior after carrying out the program of" patient's selection for the doctor".We analyse and compare the difference of judgement on the influence of " patient selection medicine " on medical worker's job behavior between medical worker and patient, between internal medicine impatient and surgical impatient, between the medical workers of general hospital and medical workers of section hospital,between administrative staff and business personnel, between the doctor and nurse, between examine doctor and non-examine doctor in a diagnosis group,between doctors participateing in a diagnosis group and ones not participateing in a diagnosis group.We carry on statistics analyse to census data with software package SPSS 10.0 for Windows, and count comparing with Wilcoxon examine; compare with the point system , that is besides personal information issue,choose 1 for 5 point, choose 2 for 4 point, choose 3 for 3 point, choose 4 for 2 point, choose 5 for 1 point. ResultAmong the medical workers investigated, the proportion of thinking that medical worker's sense of responsibility is strengthened, time to handle patients increases, attitude is improving , practising medicine honestly is improving and the sincere degree is improved are 80.4% , 72. 4%, 82.4% , 54.7% , 61.2% respectively. Among the patients investigated, the proportion of thinking that medical worker's sense of responsibility is strengthened, time to handle patients increases , attitude is improving , practising medicine honestly is improving and the sincere degree is improved is 94.5% , 83.6% , 96.2% , 89.0% , 93.9% respectively. Between the medical workers of general hospital and medical workers of section hospital, between business personnel and administrative staff, between medical workers and patients, the judgement of medical worker's sense of responsibility, time , attitude , honest to practise medicine , sincere degrees during making a diagnosis is different significantly( P<0.05). Between doctor and nurse the judgement of time to handle patients , honestly practicing medicine has difference significantly( P<0.05),and of medical worker's sense ofresponsibility, attitude , sincere degrees during making a diagnosis isn't difference (P >0.05).Between examine doctor and non- examine doctor in a diagnosis group the judgement to medical worker's sense of responsibility, time , attitude , honest to practise medicine has difference significantly( P<0.05),but of sincere degrees during making a diagnosis isn't difference (P >0.05). Between doctor participateing in a diagnosis group and ones not participateing in a diagnosis group the judgement of medical worker's sense of responsibility, time , attitude , sincere degrees during making a diagnosis isn't difference (P >0.05), but of honest to practise medicine has difference slgnificantly( P<0.05).
