Objective: 1. To study the clinical significance of changes of plasma levels of nitric oxide (NO) and tumor necrosis factor a (TNF-a) in patients with congestive heart failure (CHF). 2. To observe the relationship between the changes of these indexes and the degree of cardiac function, the primary affection, and. zheng differentiation-classification. 3. To investigate the changes of these indexes after treatment between control group and traditional Chinese drug treatment (TCDT) group, and the difference between control group and TCDT group. Methods: 1. 78 cases with CHF were divided into control group and TCDT group randomly, control group was treated by regular western medicine and TCDT group was treated by regular western medicine and Kangdaxin oral liquid. 2. Cardiac function, the primary affection, zheng differentiation-classification and amount of NO and TNF-a in blood plasma of all cases with CHF were recordedbefore and after treatment. 3.20 healthy subjects served as normal group. Results: 1. Plasma NO and TNF-a levels in patients were higher than those in normal subjects (p<0.01). The more severe CHF was, the more plasma NO and TNF-a increased. 2. Plasma NO and TNF-a levels in patients with CHF of hypertension and coronary heart disease (CHD) were not remarkable difference (p>0.05). But those of dilated cardiomyopathy (DCM) and rheumatic heart disease(RHD) were remarkable difference (p<0.01). 3. Plasma NO and TNF-a levels in patients with CHF of zheng differentiation-classification showed rising trend gradually. (Xinqiyangxu group 0.05). 4. Plasma NO and TNF-a levels in patients with CHF of control group and TCDT group were reduced after treatment (p<0.01), but the amount of NO and TNF-a of who were treated were higher than that of normal group (p<0.01). The amount of NO and TNF-a of TCDT group were lower than that of control group (p<0.05). 5. Effect of TCDT group was better than that of control group (p<0.05).Conclusions: 1. The determination of plasma NO and TNF-a contributes to judging the severity of CHF. 2. There were not relationship probably between the changes of all indexes and the different primary affection. 3. The changes of allindexes contribute to districting different zheng differentiation-classification. 4. Effect of those treated by regular western medicine and Kangdaxin oral liquid was better than those treated by regular western medicine only.
