A Clinical Experimental Study On The Effect And Mechanism Of Manshuailing Oral Liquid In Treating Patients Of Type Heart And Kidney Yang Deficiency Heart Failure Caused By Ischemic Heart Disease

A clinical experimental study on the effect of manshuailing oral liquid in treating patients of type heart and kidney Yang deficiency heart failure caused by ischemic heart disease[Abstract] Objective To investigate the therapeutic effect and mechanism of manshuailing oral liquid in treating patients of type heart and kidney Yang deficiency heart failure caused by ischemic heart disease. Methods 82 patients of heart failure were randomly divided into 2 groups, 41 cases in the routine treatment group(RT) received common therapy including digitalis, diuretics and vasodilators, while another 41 cases in the Chinese herb medicine group(CH) were treated basically as that of the above group, but with additional 200ml manshuailing oral liquid treatment, twice a day, for 4 wk. Clinical signs, symptoms and parameters of left ventricular function on ultrasonic cardiography were observed, and the plasma level of tumor necrosis factor- α (TNF- α ) , interleukin-6(IL-6),interleukin-10(IL-10) , platelet activating molecule CD62P(CD62P), platelet activating molecule CD63(CD63) and intraerythrocyte Ca²⁺ (IE Ca²⁺)which were detected by ELISA and flow cell apparatus separately, were measured before and after 4 wks' treatment. Results Comparing with that of pretreatment in the same group and that of after treatment in the RT group , the total effective rate, singular symptom score, heart failure symptomatic score according to Boston heart failure standard (HCF) , ischemic changes in cardiogram, stroke volume (SV) ,cardiac output (CO) ,cardiac index (CD ,eject fraction (EF) ,peak velocity of rapid filling in left ventricular early diastolic phase (Emax) .peak velocity of left atria systolic phase (Amass) ,ratio of Emax/ Amass (E/A) ,isovolumic ventricular relaxing time (IVRT) and thickening rate of left ventricular posterior wall (ΔT) were all improved significantly ,and the level of TNF- α ,IL-6, CD62P, CD63 and IE Ca²⁺ in the CH group were all decreased significantly(P<0.05 or P<0.01),while that of IL-10 was increased (P<0.05). Conclusion It is suggested that manshuailing oral liquid may alleviate clinical signs and improve heart function of patients with type heart and kidney Yang deficiency heart failure caused by ischemic heart disease by regulating the production of cytokins, decreasing platelet activation ,and lowering IECa2+.
