| Objective ' In 1988, Reaven found that essential hypertension patients hadinsulin resistance, hyperinsulinism, glucose intolerance and dyslipidemia. The basis was insulin resistance. At present, the medicine for essential hypertension does not improve insulin resistance. So, on the basis of the traditional Chinese medicineand Western medicine, We mixed up L-Arginine, taurine, dan shen, hirudo et al and made into capsule-"tangzhixiao". In this study , we observe the clinical effect of the drug "tangzhixiao" and research its mechanism .We hope that this study can lay a theoretical and practical foundation for exploring the drug to treat metabolic hypertension.Methods The study comprised 112 patients selected from all patients whohad seek medical advice from 2001 .3 to 2002.1 after excluding ones out of according with the experimental standard . There are 67 males and 45 females in this experiment. Scheme hi detail are as follows:(1) Diabetic group concludes 58 patients. There are 29 patients in diabetic group who were give the treatment scheme: 10 grains of "tangzhixiao" capsule, po, tid; Contrast group concludes 29 patients, whose treatment scheme is 80mg of Gliclazide tablet, po, bid. Case number of patients with abnormal blood lipid is 54 patients. 27 patients were give the treatment and the treatment scheme they were given is the same as what mentioned above; Case number of contrast group is 27, and they were given the treatment scheme :100mg of Alginric Sodium Diester tablet, po, tid. (2)According with thediagnosis standard of essential hypertension (WHO 1999), we divided these patients into diabetes and essential hypertension(EH)group, diabetes and normal blood pressure(NBP)group; abnormal blood lipid and essential hypertension(EH) group, abnormal blood lipid and normal blood pressure(NBP)group. Patients of EH group were given the treatment scheme: 10 grains of "tangzhixiao" capsule, po, tid. (3) Examinations of clinical symptoms of Diabetes, triglyceride(TG), cholesterol(TC), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), nitric oxide synthase (NOS), fasting insulin (FINS), fasting plasma glucose(FPG) , insulin sensitive index(ISI), systolic blood pressure (SBP), diastolic blood pressure(DBP), SGPT, BUN, Cr were performed preoperatively and 3 months later.Results 1 ."tangzhixiao" capsule can alleviate clinical symptoms(p<0.05).2. More effectively drop fasting plasma glucose (FPG) content from 8.30?.32 to 6.12?.30mmol/L and FINS content from 49.3+2.5 to 34.4?.0mU/L and increase ISI from -6.68+0.31 to -5.07+0.29 than the contrast (P<0.05). 3. Its function adjusting blood lipid content is to greatly decrease total cholesterol (TC), triglyceride (TG), and low density lipoprotein cholesterol (LDL-C) content from 6.88+0.54, 2.85?.31and 4.12+0.25 to 5.82+0.48, 2.01?.28 and 3.05+0.35 mmol/L, respectively. And to increase high density lipoprotein cholesterol (HDL-C)content from 1.38?.32 to 1.78 ?.31 mmol/L. 4. "tangzhixiao" capsule has no effect for SGPT, BuN, Cr. 5. This capsule can also make a notable impact on blood pressure: For diabetic group, systolic blood pressure falls from 168.6 ?15.5 to 126.7眑l.lmmHg; diastolic blood pressure from 101.3 ?10.2 to 82.4 ?8.3mmHg (P<0.01). For abnormal blood lipid group, systolic blood pressure falls from 160.86+14.35 to 126.84?.75 mmHg; diastolic blood pressure falls from 99.47?.03 to 72.03 ?9.1mmHg (P<0.01). 5. There is not different between thetwo groups on side-effect.Conclusions "TangZhiXiao" capsule can obviously alleviate symptoms, decrease FPG and adjust lipid content .In addition, it is able to lower blood pressure. And its functions mentioned above all relating to the use of FPG, drop of FINS content, improvement of ISI and the drop of peripheral blood vessel resistance.
