Objective:Survey the prevalence of diabetes of the B-thalassemia major patients guangxi; Explore diabetes pathogenesis of the β-thalassemia major patients.Methods:1. Enrolled in the study cases of s the B-thalassemia major patients-125, Determine which serum iron protein (SF), fasting glucose intolerence (FBG), fasting insulin (FINS), glycated hemoglobin (HbAlc). Through which the calculation of insulin resistance index (homa-ir), insulin secretion index (HOMA-B)to calculate relevant analysis iron protein (SF) and fasting glucose intolerence (FBG), fasting islet (FINS), insulin secretion index (HOMA-B), insulin resistance index (homa-ir) between pearson correlation analysis. Using multiple linear regression estimate Various Influential factorsthe to influence of serum iron protein and establish the regression equation. At the same time for all patients into B-thalassemia major patients with abnormal glucose and the B-thalassemia major patients with euglycemia, the two groups of between HbAlc, FINS, FBG, HOMA-B, homa-ir, use2sample T-test are compared.2. serum iron protein (SF)<2500ng/L for A group, SF≥2500ng/L,<5000ng/L for group B, SF≥5000for group C. Three groups of application completely random design the analysis of variance (One-Way ANOVA) and which do S-N-K two comparison is the difference between the various factors..Results:1.Diabetes incidence of the B-thalassemia major patients is increasing gradually with age,10-15years were treated with the highest incidence at this stage, fasting glucose intolerence rate20.9%, Incidence of Diabetes is23.1%.2.125patients of the β-thalassemia major patients glycated hemoglobin and age do Pearson correlation related analysis, r=0.24, p=000<0.01, as the growth of the age, more and more is also high hemoglobin alc, have apparent positive correlation3. Iron protein (SF) with insulin resistance index (homa-ir), fasting blood sugar (FBG) was positively associated with statistical significance (P<0.01). But Iron protein (SF) with insulin secretion was negative correlation index(P<0.01), and fasting insulin no relevant.4. The β-thalassemia major patients with abnormal glucose and the β-thalassemia major patients with euglycemia which fasting insulin, insulin secretion index, insulin resistance index have obvious difference(P<0.01), but the2groups of glycated hemoglobin was no obvious difference(P>0.05).Conclusion:1. Incidence of diabetes of the B-thalassemia major patients in guangxi was4%, fasting glucose intolerence rate was16%, the incidence of diabetes in patients with the B-thalassemia is increasing gradually with age.2. Iron overload for the cause of insulin resistance and insulin secretion decline is the main pathogenesis of diabetes.
