A Study on the Levels of Soluble Intercellular Adhesion Molecule-1 in the Gingival Crevicular Fluid of the Malocclusion Patients with Fixed Appliances.OBJECTIVE To investigate the levels of soluble intercellular adhesion molecule-1 (sICAM-1) shed into the gingival crevicular fluid (GCF) of 31 malocclusion cases, which used fixed appliances orthodontic treatment in period of pre-treatment, 1,4,8 weeks of treatment. To determine if the fixed appliance will affect the periodontal health and to provide a non-invasive method to study the condition of periodontal tissues.METHODS The soluble intercellular adhesion in gingival crevicular fluid carefully collected with standardized filter strips from 31 patients(12 men, 19 women, 12-27 years old) with fixed appliances were measured, using an ELISA technique. The periodontal index including probing depth(PD) and gingival index(GI) were recorded following carefully sampling of GCF. One week before treatment the observed value was served as the baseline records. The varying relation between the level of GCF and sICAM-1 during treatment were analyzed.RESUTTS After the fixed appliances placement significant differences were found in the levels of sICAM-K GCF, GI and PD. There were statistical differences between pre-treatment and after treatment, P<0.001. GI and PD increased significantly in period of 8 weeks of treatment. GCF increased significantly in period of one week of treatment, PO.001. There was no difference between the levels of sICAM-1 in GCF in period of one week pre-treatment and that in period of one week of treatment, P>0.0 5. After one week of treatment,the levels of sICAM-1 in GCF increased, P<0.001.CONCLUSIONS Using fixed appliances placement will affect the periodontal health. Therefore, it is very important to strengthen the oral hygiene instruction to the people who receive the treatment of fixed appliances. The periodontal adaptabilities are relation to gingival inflammation, oral hygiene and orthodontic force. It suggests that the doctors should keep periodontal health, use orthodontic force reasonably and provide orthodontic treatment regularly. The levels of soluble intercellular adhesion molecule-1 in GCF can be used as an efficient method for monitoring periodontal health during orthodontic treatment.
