Objective: To determine the roles of CD137, interleukin-5 and fibronection in the etiology of nasal polyps by immunohistochemistry. Materials and Methods: The specimens of 44 nasal polyps and of 11 control subjects were studied. 44 specimens were divided into A and B group on the basis of the endoscopic fmdings, computed tomography and the criterion of chronic sinusitis and nasal polyps classification. The slides were stained with the mouse monoclonal antibodies of IL-5, Fibronectin and CD 137 separately. A mean numbers of IL-5+ cells per high powerfiled were counted, the distribution of Fibronectin and positive eosinophils of CD 137 in selected samples were observed. Ali data were analyzed with ANOVA or X2 test using SAS software. Results: Selected polyp specimens shown abundant number of positive cells and eosinophils of IL-5 staining. The number of positive cells and eosinophils of IL-5+ in both A and B group was significantly higher than that in controls (P<0.01). Positive lymphocytes only showed a significant difference between nasal polyps and control, but was notseen between grpup A and B of nasal polyps. Positive flbronectin immunostaining was seen in the lamina propria with severe edematous changes, the difference in the flbronectin expression between the eosinophil type and the other was signiflcant, Fibronectin positivity of group A was significantly higher than these of group B (p<0.05); Positive CD137 immunostainning show a signiflcant difference between group A and B. Conclusion: 〦osinophil and cvtokine plays a?important roles in the mechanism leading to the growth of nasal polyps. Eosinophilia is the main histologic characteristic of nasal polyps.(2)The positive expression of IL-5 was seen in eosinophils, lymphocytes, plasmacytes and fibroblast cells infiltrated nasal polyps tissue. Signiflcant relation between the expression of IL-5 and eosinophil infiltration was recognized. (3) Eosinophil infiltration, edematous changes and flbronectin positivity also showed signiflcant correlation in nasal polyps. Fibronectin is an important factor in the promoting the growth of nasal polyps.(4)The tissue eosinophils of nasal polyps express CD137.The close relation was shown among the expression of CD137, the tissue eosinophila and extent of nasal polyps. This result indicates the CD 137 expression might limitGM-CSF-mediated and IL-5-mediated antiapoptosis of eosinophils.
