Diabetes mellitus (DM) has become the public health problem that is concerned by many countries all over the world. Diabetic retinopathy (DR) is one of the most common and important diabetic microvascular complications, and has close relationship with other chronic diabetic complications. As DR usually predicts how serious the pathologic changes of the blood vessels and the nervous system are, it is important and significant to study the pathogenesis and the treatment of DR in order to study the prevention and the treatment of other chronic diabetic complications.The pathogenic mechanism of DR is very complicated and mainly as follows: (1) protein non-enzymatic glycosylation; (2) activation of polybasic alcohol pathway; (3) intensive oxidation; (4) disturbance of lipid metabolism; (5) abnormal function of platelet; (6) change of blood rheology. Protein non-enzymatic glycosylation (NEG) is the basis of all the motofacient factors mentioned above. It can activate and promote other pathologic changes, and elicit many biological reactions that can harm the body. Therefore, protein NEG has strong influence on the course of DR, and is its first and basic pathogenic mechanism. There are no effective methods to completely cure DR in modern medicine. Of all the medications that are now prepared for DR, the inhibitor of protein NEG which has certain influence on diabetic microvascularcomplications attracts more and more attention in the medical field, and has showed a promising applied future.DR belongs to such internal ophthalmopathy as the poor vision, the blur vision like clouds in front of eyes and so on in Traditional Chinese Medicine (TCM). Its basic pathogenesis is as follows: (1) lack of moisture due to deficiency of yin; (2) lack of activation due to deficiency of qi; (3) stagnation of blood and qi, and involves five viscera. The characteristics of DR's pathogenesis lie in "deficiency" and "stagnation", which refer to deficiency of qi and yin and stagnation of blood. Deficiency of qi and yin is the root, while stagnation of blood is the manifestation. Deficiency of qi and yin and stagnation of blood can act on each other, so DR is developed constantly under their effects. Protein NEG closely relates to the stagnation syndrome of DR, and is one of the basic pathogenic changes of stagnation of blood, so the level of protein NEG can reflect the degree of the stagnation syndrome of DR. Directing against the characteristics of DR's pathogenesis that are "deficiency" and "stagnation", we choose San Huang Jiang Tang Pian (ingredients: Da Huang, Tao Ren, Gui Zhi, Gan Cao, Xuan Shen, Sheng Di, Mai Dong, Huang Qi, Ge Gen, Huai Shan Yao) that is from Professor Xiong Man Qi's experiential formula梞odified Tao He Cheng Qi Tang to treat DR in order to tonify qi and fluid, nourish yin, moisturize dryness and activate blood circulation to dissipate blood stasis.We choose 37 simple diabetic retinopathy cases to- study the influence of San Huang Jiang Tang Pian on protein non-enzymatic glycosylation of DR, setting the control group of Aspirin. We found that San Huang Jiang Tang Pian can lower the blood sugar and the level of glycosylated hemoglobin (HbA,c)and serum fructosamine (FTM), so it can alleviate NEG of the circulatory protein, and inhibit the production of the intermediate products of protein NEG to a certain extent. San Huang Jiang Tang Pian can reduce the glycosylated erythrocytic membrane protein, lower the total mass of erythrocytic membrane protein, so it can alleviate NEG of the structural protein, inhibit the advanced stage of protein NEG to a certain extent, and reduce the advanced glycosylated endproducts (AGEs). San Huang Jiang Tang Pian can promote the content and activity of antioxidase, decrease the production of free radical especially the lipid peroxidation (Lpo), so it can inhibit oxidation to a certain extent, and alleviate the harmful biological reactions caused by protein NEG.
