Study On Systematic Self - Efficacy Intervention Program In Patients With Chronic Obstructive Pulmonary Disease

Objective: Based on the self-efficacy theory, the chronic obstructive pulmonary disease(COPD)patients were carried out self-efficacy intervention as the guidance of systematic self-efficacy intervention scheme. Through evaluating the level of self-efficacy and health related quality of life of COPD patients, it aims at providing theoretical basis for the further implementation of the nursing intervention in patients with COPD.Methods: According to the principle of minimization,meanwhile on the basis of patient’s gender,educational level, living conditions, medical sources, monthly income, course of disease, a total of 104 COPD cases at respiratory medicine department in Shanxian Central Hospital from January 2014 to October 2014 were selected and randomly divided into intervention group and control group, which each group included 52 cases. Both groups of COPD patients were treated the conventional therapy and nursing, at the same time the intervention group was carried out systematic self-efficacy intervention as the guidance of systematic self-efficacy intervention plan on the basis of the conventional treatment, which lasted 16 weeks. Before and after intervention, the level of self-efficacy,pulmonary function, health related quality of life(HRQOL), blood gas analysis values, 6 minutes walking distance(6 MWDS) and dyspnea of the medical research council(MMRC) of each patient were evaluated respectively. The general information questionnaire was used to collect basic data and pulmonary function, blood gas analysis values below quiet condition, 6 minutes walking distance and other information of COPD patients; Self-efficacy scale developed by Kate Loring, Quality of life scale about adult chronic obstructive pulmonary disease and dyspnea of MMRC scale were used to measure the level of self-efficacy, HRQOL and exercise endurance of COPD patients respectively. It used SPSS17.5 software package for data analysis.Results: The level of self-efficacy, HRQOL scores, Pa O2, Pa CO2, Sa O2, 6 MWDS in intervention group were 43.45±6.84 points, 55.59±25.71 points, 68.28±8.51 mm Hg, 45.14±10.14 mm Hg,92.82±2.48 mm Hg, 274.73±81.29 m respectively. However, corresponding to each measure, it was40.57 ±3.43 points, 74.41±7.63 points, 59.57±10.42 mm Hg, 52.17±11.31 mm Hg, 91.18±2.37 mm Hg,269.29±57.13 m respectively in control group. There were significant differences between the intervention group and the control group(P<0.05); The pulmonary function in the intervention group was improved, but it didn’t exist significant statistical differences between the two groups(P>0.05).Conclusion: Systematic self-efficacy intervention can effectively improve the ability of self-efficacy and health related quality of life in the patients with COPD.
