Objective:How to improve the surgical treatment efficacy of Bowen’s disease or squamous cell carcinoma when there is no condition to Mohs micrographic surgery (MMS).Methods:Photodynamic diagnosis has been made on 8 patients who have been diagnosed of Bowen’s disease and 8 patients who have been diagnosed of cutaneous squamous cell carcinoma. These patients’skin test are all negative. Before the application of a photosensitizer, excessive scales and crusts were gently scraped off without bleeding and the lesions were cleansed using physiological saline gauze.20% ALA was then applied over the tumor lesions, including a 0.5 to 1 cm area of surrounding skin. The area was covered with polyurethane film and aluminum foil to prevent exposure to light. After incubation times of 6 hours, the occlusive dressing was removed, and the lesion was cleansed with saline to remove excess photosensitizer cream. To determine the margin of primary cSCC and Bowen’s disease, the red fluorescence was marked with gentian violet under a Wood lamp. Mark the fluorescent signal distribution and determine the extent of resection. Each of the PDD and non-PDD groups was surgically staged and compared.Results:There is 0 case whose histopathology is positive in the PDD group and 8 cases in the non-PDD for Bowen’s disease; there are 2 case whose histopathology is positive in the PDD group and 6 cases in the non-PDD for cSCC.Conlusions:In the present study, the positive rate of histopathology of the PDD group and the non-PDD for both Bowen’s disease and cScc shows no statistically significant difference of the PDD support surgical efficacy for Bowen’s disease and cSCC.
