Preliminary Clinical Observation On 20 Cases Of Diabetic Peripheral Neuropathy Treated By Wutou-honghua-guizhi Leg & Foot Bathing Method Combined With Lipoic Acid

The thesis consists two parts:the literature review and the clinical research.There are 2 parts of the literature review.The first one reviews the real application of traditional Chinese medicine Leg&Foot Bathing Method in the treatment of Diabetic Peripheral Neuropathy.The research is undertaken in two direction,one is based on local treatment with syndrome differentiation,the other is based on general treatment with syndrome differentiation.The former includes the constant prescription,the constant prescription added or subtracted,the constant prescription combined with Traditional Chinese medicine decoction,or combined with Western medicine,or combined with Traditional Chinese medicine injection,and the constant prescription combined with acupuncture or massge.the later is choosing different Leg&Foot Bathing prescription which is based on general treatment with syndrome differentiation.This paper also tries to figure out the characteristics of Leg&Foot Bathing Method drugs,finds the effective mechanism of the treatment of Diabetic Peripheral Neuropathy under Leg&Foot Bathing Method,and promote the extensive prospect and the popularizationvalue of this method.The second one is about the cause research and treatment method of the disease in recent years.Because of the complex mechanism,which relates to hyperglycemia that causes the metabolic disturbance、the microcirculation disturbance、the excessive oxidative stress、the lacking neurotrophic factors、 the disorder autoimmunity and so on.So,this part mainly sets forth every kind of medicine,which corresponds some kind mechanism,although every kind of medicine has some clinical efficacy,but the total result is not ideal.The part of clinical research, Objective to observe the research of efficacy for treating DPN with Wutou-honghua-guizhi Leg&Foot Bathing prescription combined with lipoic acid.Methods Firstly.after having been signed the informed comsent,40 DPN patients conforming to the diagnostic criteria were randomly divided into the convention combined with Leg&Foot Bathing group(20 patients)and the conventional group(20 patients).The two groups all used hypoglycemic drugs、traditional Chinese medicine decoction and lipoic acid,on the basis of the above treatment,the convention combined with Leg&Foot Bathing group was given Chinese medicine Leg&Foot Bathing Method.and the Bathing prescription is Wutou-honghua-guizhi soup.The couse of the treatment was two weeks,the changes in the symptoms、sign and the blood sugarwere observed in the patients,the corresponding form was set out,which was the main outcome measurement and the second outcome measurement.Secondly,12 patients coming from the convention combined with Leg&Foot Bathing group continued to accept consolidation treatment for 2 weeks after the couse of the treatment,those patients formed the follow-up group. Results the total effective rate of the convention combined with Leg&Foot Bathing group was 95.0% and that of conventional group was 65.0%,By the chi-square test,the effect of convention combined with Leg&Foot Bathing group was better than the conventional group, the difference was very significant (p=0.048,<0.05).And the results also showed that the convention combined with Leg&Foot Bathing group and the conventional group had obviously difference (p=0.006<0.01) in symptoms, specially the symptoms of pain, cold of lower Libs and abnormal feeling through your fingers.But they had indistinctive difference (p=0.134>0.05) in sign.There were no obvious side effects found during the treatment.Conclusion Wutou-honghua-guizhi Leg&Foot Bathing prescription combined with lipoic acid could obviously improve the clinical curative effect,the unique advantage of Chinese medicine Leg&Foot Bathing probably was to relieve the symptoms of patients,Curative effect and treatment were positively correlated. It indicated that comprehensive treatment based on the Chinese medicine Leg&Foot Bathing Method had a clear advantage,the Bathing prescription of Wutou-honghua-guizhi soup deserves further research.
