Analysis On The Function And Management Of Health Supervision In Yangpu District

[Objective]Health inspection is that the health administrative departments maintain the public health and medical service order, and protect people’s health and rights, and cause directly legal effect on specific taken citizens, legal persons and other organizations by national health laws or regulations. The function is:health administrative license, health inspection, health monitoring and evaluation, case investigation, handling complaints, emergency disposal, and health safeguard of important activities, health education, information management, inspection team training and management.It is a complicated system engineering that evaluates the work of health inspection. We must have a scientific evaluation theory, which can evaluate of the full range in health inspection resources, function and satisfaction. The existing evaluation methods used in health inspection are:①"Structure"-"Process"-"Results" Evaluation Theory. From both the practice and theory, this can objectively and comprehensively analyze and evaluate the existing condition of health inspection mechanism. After identify "what to do", we can analyse "How is doing", and find out the gap and deficiency which help the further suggestions and measures came out for the next step of work.②Competency Model Theory. It widely is used for talent review, works as basis and guidance of human resource management, but can not evaluate for functions and values about the whole institute.③Comprehensive Evaluation Method. It is by the conventional method, fuzzy mathematics method and multiple statistical methods, index to the standard value, and then to the synthetically evaluation index, after that carry out sorting, comparison, analysis. Like the Topsis, it can be used in the compare of different period in same district, and different district in same period, but this method needs high quality mathematical statistics analysis. We should choose the idiographic&intuitionist evaluation considering the actual condition, relationship between the indicators, as well as the influence of confounding factors and those need to chosen by selecting the correction index&revising evaluation result. Besides the above evaluation theory and method, according literature record, there’s some correlative analysis way to evaluate the health inspection of different district. From the existing literature, many studies focused more on assessment of health inspection system, quality and service, more about how to set up the system of health inspection, how to build the management mode of health inspection, as well as the research and analysis in the allocation of resources, staff performance and inspection satisfaction degree. No literature records about the evaluation of regional and comprehensive ability. Therefore, this research via the historical review of nearly two years of Yangpu health inspection, including the allocation of resources, administrative licensing, the daily inspection and satisfaction evaluation, knowing the health inspection system and the practice situation of Yangpu. Then we using "structure-process-result" evaluation theory, understand what the current situation is, what institution construction and function implements state of the Yangpu health inspection is, and what factors affect the implementation of its functions, how to improve the implementation to ensure the function, To sum up, the study offers the decision basis to improve health supervision management area, which can adapt to the development of regional social economy.[Methods]First, the writer collected all kinds of literatures about evaluation for health inspection and management function, analyzed the health inspection function, evaluation method and what is the real status now. Second, the writer investigate in Yangpu health inspection institutions through method of statistical data&extraction, collect data of the mechanism of human resources, equipment and facilities allocation as well as how work fund used in2010～2011; investigate in disinfection products, drinking water, schools and kindergartens, occupation and radiological health, medical practice, unlicensed medical investigation and banned, as well as daily inspection, investigation and complaint data. Under the guidance of the principle "structure-process-result", the writer explore the allocation of resources, management&fulfilling functions through corresponding analysis, descriptive analysis and other statistical methods. Meanwhile, carry out the external evaluation, satisfaction questionnaire survey to the relevant departments of the relative person of management and administration. Finally, the writer organize relevant experts to discuss and clear the current status of health inspection and management of Yangpu District, what are the main shortages, what are the root reasons, and the experts also give the suggestions to improve.[Results]The study shows Yangpu health inspection institute only represent for health administrative department to enforce the law, but still separation from the subject of law enforcement and law enforcement agency, although the institute go through several adjustments and innovations. This also results in the separation of duties, overlapping functions, unclear division of work and administrative intervention in Yangpu health inspection whose supervision still not smoothly. Besides, though the financial investment increased year by year, the allocation and guarantees of resources still not enough. Of course, in recent2years, the human resource, material and financial resources are obviously increased, but the financial resources still not enough because of local government have relatively litter concern on health inspection.The study also shows the ability of health inspection gradually increased via innovative supervision, but there is uneven phenomenon in the daily inspection. One good aspect is the inspection strength to reach100%in the centralized water supply, the water supply pipeline, and disinfection products, to reach over95%in swimming pools, two water supply, school health inspection. Contrarily, the cover rate of regulation is less than50%in beauty salons, foot bath and chess, small food and beverage, nurseries and kindergartens, some even less than30%.The study shows the administrative punishment decreased smoothly, but still need to improve the quality of the case. The new or complicated cases of Yangpu health inspection are behind of Shanghai average and health supervisor’s ability need to be improved, shows the perseveration in cracking down the illegal medical practice, and the complaint report processing remains the priority among priorities."government-department-the linkage" this kind of mode, strengthen supervising in the practice of medicine without a license. But, professional medical complaint rate remains high, meanwhile, drinking water health complaint report also showed an upward trend.The study also shows Yangpu health inspection institute is recognized by public from the result of the external evaluation. But the relative persons of management not satisfied with the complaint report, propaganda and training. The satisfaction rate is only close to50%in public places, drinking water hygiene and social awareness from the government.The study suggest that Yangpu health inspection should strictly follow government innovation, straighten out supervisor relations and mechanisms as soon as possible; should enhance the supervision, improve the comprehensive quality of health inspection team by strengthen management; should increase the daily inspection coverage and quality control by the innovation methods.; should improve fulfill rate of reported complaint and the ability to deal with emergency; Should strengthen polarization, have more trainings about the laws and regulations to the team. Yangpu government shall pay more attention on health inspection, ensure more resources invested in.
