Abstract/Summary:
ObjectiveIn this study,patients with lumbar disc herniation(LDH)were taken as the research target.On the principle of selecting the local effective points for the treatment of lumbar disc herniation,the depth of needling was adjusted to observe the clinical effect of deep needling on the treatment of lumbar disc herniation,improving the clinical effect of traditional acupuncture on the treatment of lumbar disc herniation.MethodsIn this study,patients with lumbar disc herniation were recruited from Hong Kong clinic from January 2021 to December 2021.A total of 60 LDH patients who met the inclusion criteria were included in this study.By simple randomized controlled method,the patients were divided into the deep needling group and the conventional needling group with 30 cases each.1.Treatment plan:The second to the fifth Jiaji points and BL23-26 were selected from lumbosacral parts.The location of acupoints refers to the National standards of The People’s Republic of China "Nomenclature and location of meridian points" and "Nomenclature and location of extra points in common use".In the deep needling group,0.25x75mm needle was used for direct needling around 40~70mm.The degree of needling sensation "Deqi" should be transmitted to lumbar buttocks,groin,inner thigh and has "meridian sensation transmission" to the lower limbs.In the control group,0.25x40mm needle was used with the same acupoint.The depth of needling was around 10-3 0mm.The degree of needling sensation "Deqi" was restricted around the needling area.Needles were retained for 30min after the treatment in both groups.2.Course of treatment:Both groups received acupuncture treatment for 6 weeks,once a week,a total of 6 times treatments in one course.3.Observation indicators:Visual analogue Scale(VAS),Japanese Orthopaedic Association assessment treatment score(JOA)and Oswestry disability index(ODI)were compared between the two groups before and after the treatment to compare the difference in efficacy between the two groups.4.Statistical methods:SPSS 26.0 software was used for data analysis in this study.First,the clinical data was set up in a database for statistical processing and analysis.Shapiro-Wilk test was used to check the data with normal distribution or not.T-test was used for sample with normal distribution.The paired sample T-test was used for intra-group comparison and the two independent sample T-test was used for inter-group comparison.Nonparametric test was used for measurement data that did not conform to normal distribution.Wilcoxon signedrank test was used for intra-group comparison,and the Mann-Whitney U test was used for inter-group comparison.Logistic Regression and Spearman correlation analysis were used for data correlation analysis.Chi-square test was used for counting data.P<0.05 was considered statistically significant,while P<0.01 was considered highly statistically significant.Results1.Baseline data:baseline data indicated that there were no statistically significant differences in age,gender and course of disease between the two groups,P>0.05.VAS,JOA and ODI data were compared before the treatment.The datas were not normally distributed between the two groups,and the scores of all indicators were consistent at baseline when the patients were enrolled,with P>0.05,showing no statistical significance with good comparability.2.Primary outcome indicators:In the evaluation of the analgesic effect,the improvement rate of VAS score of the deep needling treatment group was better than that of the conventional needling control group with statistical significance(P<0.05).In terms of improving lumbar function,the improvement rate of the JOA score of the deep needling treatment group was better than that of the conventional needling control group,with higly statistical significance(P<0.01).3.Secondary outcome indicators:In the efficacy evaluation of the secondary outcome indicators,the improvement rate of the ODI score of the deep needling treatment group was better than that of the conventional needling control group.Deep needling has a better result in improving the quality of life of LDH patients,with highly statistical significance(P<0.01).VAS,JOA and ODI were positively correlated(P<0.01)in determining the correlation of all indicators.In terms of the relationship between LDH course and VAS improvement rate,there was a highly significant correlation between them in both groups(P<0.01).4.Efficacy evaluation:In the treatment group,93.3%,86.7%and 96.7%of VAS,ODI and JOA improvement rates were assessed as effective respectively.In the control group,66.7%,56.7%and 66.7%of VAS,ODI and JOA improvement rates were evaluated as effective respectively.ConclusionIn the treatment of lumbar disc herniation,deep needling can effectively relieve the pain,improve the lumbar function of patients,improve the quality of life of patients,and the curative effect is better than that of the conventional acupuncture control group.The relief of the pain was correlated with the recovery of lumbar function and the improvement of life quality.Deep needling has a certain effect on the treatment of LDH,and the social benefit of medical service is greater,which is worth promoting to the clinical practices.
