| Object:By contrasting clinical and biochemical index among the overweight patients of PCOS to threatened abortion in the first trimester,the lean patients of PCOS to threatened abortion in the first trimester and healthy controls and screening of their possible differences metabolites and observating the effect of Zhu Yunning Ⅱ Decoction,exploring the pathogenesis of overweight and lean patients of PCOS to threatened abortion in the first trimester providing the basis on metabonomics for clinical diagnosis and intervention early.Materials and Methods:Frist Experiment:conforming to the inclusion criteria,the patients were divided into healthy control group 26 cases(group A),the lean patients of PCOS to threatened abortion in the first trimester group 26 cases(group B),the overweight patients of PCOS to threatened abortion in the first trimester 29 cases(group C).for patients of group B and C who received therapy with TCM of "Zhu Yunning II Decoction" oral for 4 weeks to tonify the kidney and spleen,observed the concentrations of serum E2,P,β-HCG,T,DHEAS,AND,SHBG,LDL,HDL,TG,TC,FPG,FINS and IR before and after treatment.The results of serum were tested by ELISA method,all the data were analyzed by SPSS 19.0 software.Traditional Chinese medicine of ZHU YUN NING No.Ⅱ curing patients with PCOS threatened abortion had good clinical effect.Second Experiment:The serum samples and groups were as same as the frist Experiment.The application of metabonomics is to screen metabolites of the overweight patients of PCOS to threatened abortion in the first trimester and the lean patients of PCOS to threatened abortion in the first trimester by UPLC-MC through multivariate statistical analysis providing the basis on metabonomics for clinical diagnosis and predicating early.Result:Frist Experiment::Before treatment:The levels of serum E2,P,β-HCG of group A were significantly higher than group B and group C(p<0.05),the level of E2 in Group C is significantly lower than B,levels of P and HCG between the two groups have no statistical differences C(p>0.05);The levels of serum T and AND of group B and group C were obviously higher and SHBG was lower than group A(p<0.05),the level of D H E AS had no significant difference among the three groups(p>0.05),the levels of serum AND in group B were higher than group C,there were no significant difference between group B and group C in T and SHBG(p<0.05).The levels of serum LDL、TG、TC of group C were significantly increased than group A(p<0.05),the level of HDL was significantly reduced than group A(p<0.05),The level of serum TC of group B were significantly increased than A(p<0.05),the level of HDL was significantly reduced than group A,the difference is statistically significant(p<0.05),group B and group C only have significant differences on level of LDL and TG,group C higher than that of group B(P<0.05).The levels of serum FBG,FINS and IR of group C were significantly increased than group A and group B(p<0.05),there was no statistical difference between group A and group B(p>0.05)but according to the data,there were a trend of decrease.After treatment:the level of HDL in group C was lower than that in group B and group A,other indexes had no statistical difference among the three groups(p>0.05).Second Experiment:Based on the differences of screen variables,metabolites were identified 9 differences,mainly involved in lipid metabolism,carnitine metabolism and phospholipid metabolism.PCOS threatened abortion group and control group in metabolic spectrum is different obviously,speculated that LPC 16:0sn-1、FFA 18:1、LPC 18:0 sn-1、CarnitineC2:0、LPC 1 6:0sn-2、LPC 1 8:1 sn-1、CarnitineC 10:0 for the potential biomarkers of the lean PCOS to threatened abortion in the first trimester;Speak to the LPC 16:0sn-1、FFA 18:2、FFA 18:1、CarnitineC2:0、LPC 18:0sn-1、LPC 16:0sn-2、PC32:1、CarnitineC10:0 for the potential biomarkers of the overweight PCOS to threatened abortion in the first trimester;But there were no significant difference between group the lean PCOS to threatened abortion and the overweight PCOS to threatened abortion in metabolite.Conclusion:Based on clinical and biochemical indexes,and Zhu Yunning ⅡDecoction can effectively adjust the overweight and lean PCOS patients to threatened abortion of endocrine function and blood lipid metabolism,promote the growth and development of embryo,and has a good role of fetus protection.Metabonomics are concluded,the PCOS patients to threatened abortion mainly involves many abnormal metabolic pathways including lipid metabolism、carnitine metabolism and lysophosphatidylcholines providing the basis on metabonomics for clinical diagnosis and predicating early for threatened abortion resulted from PCOS... |
