Research On The "Dual Path" Of Basic Medical Security System To Promote The Health Of The Elderly

Life and health are the basic rights and interests of all people.Medical security is an important livelihood guarantee system to defuse the economic risks of diseases and improve people's health.In August 2021,the national health insurance bureau,Ministry of Finance jointly issued by the "about the opinions of the build system of health care treatment listing",defined the boundary of the basic medical insurance system,which contains the basic medical security system was the core of basic medical insurance system,supplementary medical insurance system in a supplementary role,and the palm of medical assistance system.The basic medical security system not only contains the economic compensation function of medical expenses,but also has the broader function of improving national health.Therefore,the medical expense compensation of basic medical security system can be defined as "single path" security function,and "economic compensation + health promotion" can be defined as "double path" security function of basic medical security system.Based on the definition of the basic medical security function of "economic compensation + health promotion",this thesis carries out the "dual path" research of the basic medical security system to promote the health of the elderly population.The so-called "dual path",that is,the basic medical security system to promote the health of the elderly population,not only through the medical cost compensation function(referred to as the "medical path"),but also through the health promotion function(referred to as the "health path").The elements of "medical path" include improving "medical service utilization rate",reducing "medical expenditure",changing "medical treatment behavior" and improving "medical service timeliness",etc.The elements of "health path" include improving "health service",promoting "nutrition balance",promoting "healthy behavior" and promoting "psychological stress reduction".The basic medical security system mainly influences the health of the elderly population through the eight internal factors(mediating factors)of the above-mentioned "dual paths" of medical and health care.According to the seventh national census,the number of elderly people aged 60 and over reached 264 million and 190 million respectively,accounting for 18.7 percent and 13.5percent of the total population,up from 13.26 percent and 8.87 percent during the sixth general Population Period.In June 2021,the National Healthcare Security Administration released the Statistical Bulletin on the Development of National Medical Security in 2020,which showed that the hospitalization rate of urban employees' medical insurance retirees in China was 36%,and the medical expenses of retirees accounted for 50.03% of the total medical expenses of urban employees' medical insurance participants.The burden of medical expenses and the pressure of medical insurance fund brought by the aging population are increasingly heavy.The above data also show that under the background of deep population aging,the "single path" guarantee of basic medical security system is not conducive to the realization of healthy aging goal,the pressure relief of basic medical security fund and the sustainable development of the system.The transformation from the traditional "single path" security to the more comprehensive "double path" security mode should be the direction of the reform of the basic medical security system in the new era.This thesis takes medical treatment and health care "dual path" as the main line and consists of eight chapters.Chapter one to three are introduction,theoretical basis,arrangement of basic medical security system and local practice.The fourth and fifth chapters are the main content of this thesis,that is,the mathematical empirical analysis and survey interview of "dual paths" of basic medical security system to promote the health of the elderly population;Chapter six is the international experience of "dual path" of basic medical security system to promote the health of elderly population.The seventh chapter puts forward the policy conception of optimizing the basic medical security system from the perspective of "dual path".The last chapter is the research conclusion and prospect.This thesis comprehensively uses research methods such as text analysis,social investigation and mathematical demonstration,and takes the promotion of the health of the elderly population by the basic medical security system as the core research topic,and draws the following conclusions.First,the basic medical security system has a significant impact on the health of the elderly population through eight functional elements(mediating factors)contained in the "dual path" of medical treatment and health care.Second,the health promotion effect of basic medical security system on urban and rural areas,different income groups,different age groups and different genders of elderly population has heterogeneity.Thirdly,the "medical path" and "health care path" of the basic medical security system to promote the health of the elderly population are different successively,and there are also different primary and secondary.From the time series of the content of basic medical security system,"medical path" is first,"health care path" is second;At present,"medical path" is still the main path of health promotion,while "health care path" is the secondary path of health promotion.At different stages,the health promotion intensity of the two paths is in dynamic change.Fourth,healthy aging is a systems engineering,and to promote the elderly population health,achieve the goal of healthy aging,such as system design focuses on the basic medical security system itself,there will be limitations,should leverage on basic medical security system,play a leading role,the integration of various resources,the aggregation of multiple power,The "Big Health" strategy,which focuses on improving national health in an all-round and life-cycle way,has been designed at the top level and implemented in a comprehensive way.In this thesis,based on the aging of the population age background,healthy aging target and policy guidance to the construction of "healthy China","medical path" is put forward at the core of the concept of "health care path","double path" basic medical security system to promote the elderly population health academic views and "double path optimization policy idea of basic medical security system,Innovation in core concept,academic viewpoint and policy design;The research theme is consistent with the national strategic orientation of integrating health concepts into various public policies,insisting on prevention first,comprehensively promoting healthy aging and the construction of "Healthy China".At the same time,how to accurately define the responsibility boundary of basic medical security system and other types of health security system,There is still room for further research on how to lead other health security systems to promote the health of the elderly population through the adjustment of basic medical security policies,and how to exert the synergy of various health security systems to improve the health of the elderly population.
