Objective To systematically summarize Professor Ba Yuanming's clinical experience,medication characteristics,syndrome differentiation rules,core prescriptions and academic thoughts in treating IgA nephropathy based on syndrome differentiation.Based on the core prescription,the molecular mechanism of core prescription in treating IgA nephropathy was preliminarily explored by using network pharmacological research methods.In order to provide some reference and guidance for the clinical differentiation and treatment of IgA nephropathy,as well as for the discovery of new drugs and mechanism of the study to provide a certain reference.Methods Collect the electronic medical record information of patients with IgA nephropathy who attended the outpatient clinic of Professor Ba Yuanming of the Department of Nephrology of Hubei Provincial Hospital of Traditional Chinese Medicine from January 2018 to December 2020.With the help of the distribution statistics,association rules,cluster analysis,CART decision tree algorithm and complex network analysis methods in the ancient and modern medical case cloud platform V2.3.5 and IBM SPSS Modeler 18.0,mining and summarizing the characteristics,law of syndrome differentiation,core prescriptions and academic thoughts of Professor Ba Yuanming's treatment of IgA nephropathy.At the same time,conduct network pharmacology research on the core prescriptions that are condensed.First of all,this study intends to maximize and most comprehensively collect IgA nephropathy related genes.In addition to searching for IgA nephropathy related genes in the GWAS Catalog database,OMIM database and Gene Cards database,IgA nephropathy related genes are also searched through GEO database and pubmed literature.The genes found are regarded as genes related to IgA nephropathy.Secondly,the active ingredients of 11 traditional Chinese medicines in core prescriptions including Astragalus,Poria,Chinese Yam,Codonopsis,Cornus,Golden Cherry Seed,Gorgon,Dioscorea nigra,Imperata cylindrica,Madder,etc.,were screened through the Chinese Medicine System Pharmacology Database to meet Compounds with oral bioavailability(OB)? 30% and drug-like properties(DL)? 0.18 are regarded as active ingredients of the drug.And use the traditional Chinese medicine system pharmacology database to predict the corresponding genes of the screened active ingredients,and standardize the names of the predicted genes through the Unitprot database.Then,by combining the IgA nephropathy related genes and the core prescription related genes,the common genes for the core prescription treatment of IgA nephropathy are obtained,and the common genes are introduced into the Cytoscape 3.7.2 software to construct the visualization network of "drug-component-target-Disease" of the core prescription in treating IgA nephropathy.In addition,the common genes were introduced into the STRING database to construct a protein-protein interaction network.The cyto Hubba plug-in built in Cytoscape 3.7.2 software was used to screen out the key genes for the core prescription treatment of IgA nephropathy.Finally,the common genes for the core prescription treatment IgA Nephropathy are imported into the DAVID database for the GO and KEGG pathway enrichment analysis of the genes.Results In this study,a total of 517 electronic medical records that met the inclusion criteria were collected,and a total of 108 patients were enrolled.Among them,68 were female patients and 40 were male patients,with an average age of 38.09±10.92 years.In the statistics of TCM syndromes,there are mainly Qi and Yin deficiency syndromes,liver and kidney yin deficiency syndromes,and Xiajiao damp-heat syndromes.Professor Ba Yuanming uses calm,cold and warm drugs for the treatment of IgA nephropathy.The five flavors are mainly sweet,bitter,and pungent drugs.The meridians mainly involve the spleen,lung,kidney and liver channels.According to the efficacy of the drugs,the drugs were classified,and the results found that the drugs were mainly tonic drugs,clearing heat drugs,and diuretic and dampening drugs.Based on the association rules of the Apriori algorithm,the number of co-occurrences of two drugs is 10,and the number of three drugs that co-occurs is 14.The main drugs involved are Astragalus,Poria,Dioscorea nigra,Chinese Yam,Cornus and Codonopsis.Through cluster analysis,it was found that Professor Ba Yuanming's prescriptions for the treatment of IgA nephropathy mainly grouped into 4 categories,which contained the meanings of Shenqi Dihuang Decoction,Zhibai Dihuang Decoction,Ermiao Situ Decoction,and Sancai Fengsui Pill.With the help of the CART decision tree algorithm,seven important predictor variables,including crimson tongue,five upset fever,hematuria,sore throat,proteinuria,fatigue,and pulse length,were screened out,and a decision tree with a node of 18 and a depth of 5 was obtained.Explore the law of Professor Ba Yuanming's syndrome differentiation of IgA nephropathy.In addition,using complex network analysis,the core prescription composition of Professor Ba Yuanming's treatment of IgA nephropathy was condensed: Astragalus,Poria,Chinese Yam,Codonopsis,Cornus,Golden Sakura Seed,Gorgon,Dioscorea nigra,Imperata Root,Madder.In the network pharmacology analysis of core prescriptions,37,296,1131,107 and 204 IgA nephropathy-related genes were selected from the GWAS Catalog database,OMIM database,Gene Cards database,GEO database,and pubmed literature.After integration and removal of duplicates 1382 disease genes were obtained.194 genes were screened in the core prescription,involving 99 active ingredients.The 10 core targets for the treatment of IgA nephropathy in core prescriptions were screened through the protein interaction network,namely TNF,IL6,VEGFA,AKT1,MMP9,PTGS2,MYC,JUN,CASP3 and TP53.Through GO enrichment analysis,the biological processes involved mainly include: positive regulation of RNA polymerase II promoter transcription,drug response,inflammatory response,positive regulation of DNA template transcription,positive regulation of smooth muscle cell proliferation,and cellular response to lipopolysaccharide Etc.In addition,the results of KEGG enrichment analysis show that the active ingredients in the core prescription have a regulatory effect on the various pathogenic microorganism infections signaling pathways,TNF signaling pathways and HIF-1 signaling pathways.Conclusion According to the results of data mining,Professor Ba Yuanming's experience in treating IgA nephropathy was sorted out,and Professor Ba Yuanming 's academic thoughts on the treatment of IgA nephropathy were summarized: bring into correspondence with pathogenesis,pay attention to the treatment of IgA nephropathy by stage.1,early stage: deficiency is more common in the kidney disease and the Yin deficiency is the most;Treatment mainly to nourish Yin and tonic essence,Yin deficiency and fire flourishing syndrome should be treated by nourishing Yin and clearring heat;2,middle stage: Qi and Yin deficiency,damp heat connotation: Treatment of Qi and Yin deficiency mainly to supplement Qi and nourish Yin;Treatment of dampness and heat connotation mainly to eliminat dampness and heat;3,advanced stage: Yin and Yang are both deficiency,turbid poison stasis;Treatment mainly to Warm and nourish the spleen and kidney,cheer up Yang Qi,and release dampness drain turbidity.In this study,data mining research on the medication rules of Professor Ba Yuanming in the treatment of IgA nephropathy at the macro level was connected with the research on the molecular action mechanism of TCM compounds at the micro level,and TCM clinical data was viewed from three-dimensional and multi-angle,in order to provide certain reference for the spread of TCM clinical experience and the discovery of new drugs and clinical medication.
