| Objective:1.To use the ancient and modern medical case cloud platform to excavate the medication prescriptions of Prof.Wang Ping for the treatment of insomnia with anxiety state and summarize the clinical experience.2.To analyze the core medication prescriptions of Prof.Wang Ping for the treatment of insomnia with anxiety through the method of network pharmacology and discuss its mechanism of action.Method:1.The medical records of patients attending the specialist outpatient clinics of Hubei Chinese Medicine National Medical Hall and Hubei Provincial Hospital of Traditional Chinese Medicine from March 2021 to November 2021 were collected from Prof.Wang Ping,and the data were standardized and imported into Gujin Medical Case Cloud Platform(V2.3.5)for statistical analysis,association rule analysis,and hierarchical cluster analysis,and Prof.Wang Ping’s clinical experience in medication use was summarized.2.Conduct network pharmacology analysis based on the core drugs obtained from data mining,screen the active ingredients and action targets of the core drugs,obtain the common targets of insomnia with anxiety symptoms,construct drug-ingredient-target maps using Cytoscape 3.9.0,and construct protein interaction network maps through String database.GO analysis and KEGG pathway enrichment analysis were performed by Metecape on disease targets to explore the mechanism of action of core medication prescription on insomnia with anxiety state.Conclusion:1.119 first prescriptions and 86 repeat prescriptions were included.There were 46 males and 73 females,of which 23.52% were young adults,56.3% were middle-aged,and 16.81% were elderly.The most common symptoms of insomnia were "difficulty in falling asleep","easy to wake up at night" and "difficulty in falling asleep after waking up",accompanied by mental symptoms such as fatigue,forgetfulness and irritability.The most common types of symptoms are the heart and mind not nourished,yin deficiency and fire,heart and spleen deficiency and heart and liver fire.A total of 191 herbal medicines were involved,and the most frequently used drugs were: flat,warm,cold;sweet,bitter,pungent;liver,heart,and lung meridians;nourishing the mind,astringent,and blood;the top 10 drugs were sour date palm,roasted licorice,Chuanxiong,ginseng,Zhi Mu,prince’s ginseng,Fu Shen,Sha Shen,Dan Shen,and calcined oyster.2.The Chinese medicines with a frequency of more than 75% in the data mining were selected as the core drugs(sour date palm,roasted licorice,Chuanxiong,Zhi Mu,and ginseng).There were 145 active ingredients and240 action targets for all core drugs.There were 455 common targets for insomnia with anxiety state,and 73drug-disease action targets were obtained by taking intersection with drug targets,among which key active ingredients were kaempferol,quercetin,naringenin,isoflavonol,β-sitosterol,etc.The protein interaction network had 73 nodes and 595 edges,and the top 10 key targets were IL6,AKT1,TNF,IL1 B,CASP3,VEGFA,PTGS2,PPARG,CAT,CXCL8.GO functional enrichment analysis had 20 entries for biological processes,14 entries for cell composition,and 19 entries for analyzed functions.140 pathway signals were obtained from KEGG pathway analysis.Study Analysis:1.data mining analysis obtained at the insomnia with anxiety lesion site mainly in the heart and liver,the pathogenesis is the deficiency of vital energy,heart loss of nourishment,liver loss of drainage.To summarize Professor Wang Ping’s clinical experience in using medicine,he has been able to cultivate the vital essence,harmonize yin and yang,treat the heart and liver together,relieve depression and distress,regulate qi and blood together,and protect yin and fluids,etc.2.The network pharmacology study showed that the mechanism of action of insomnia with anxiety state may be related to the active components such as kaempferol,quercetin,naringenin and β-sitosterol,which regulate the neuroactive ligand receptor interaction pathway,c AMP signaling pathway and 5-HT signaling pathway through IL-6,IL-8,PPARG and other targets. |
