Objective:To study the risk factors of postoperative recurrence of perianal abscess with a view to identifying high-risk patients,strengthening follow-up management of patients,and providing scientific basis for improving the prognosis.Methods:1.A systematic review:The study on risk factors of postoperative recurrence of perianal abscess from December 31,2018 to December 31,2018 was included in the literature.The two researchers were independently evaluated,screened and cross-checked after inclusion in the literature by the inclusion and exclusion criteria.NOS score was used to evaluate the quality of literature.Review Manager 5.3 software,OR value and 95%CI were used for data analysis and statistics.Heterogeneity analysis was judged by I~2and Q test.2.Retrospective cohort study:In this retrospective cohort study,a total of 790 patients diagnosed with perianal abscess were enrolle from January 2015 to March 2016.The follow-up period was March 2018.The dataweb electronic data acquisition system was used to collect the patient's baseline data and TCM syndrome data,and to construct a digital and structured database.Cox model,GAM model,Interaction and stratified analyses were used to estimate the association between exposure factors and the recurrence riskResults:1.A total of 139 articles were searched and 10 studies were included in the systemevaluation according to the inclusion and exclusion.After using quantitative and qualitative analysis,20<age<40(years),poor lifestyle,comorbidities,Crohn's disease,diabetes(Chinese population),previous history of abscess,intersphincteric abscess,ischiorectal abscess,intestinal flora and incision and drainage were risk factors for recurrence of abscess;male,time from disease onset to surgery?7 days,Charlson comorbidity index 1-3 were protective factors for recurrence of abscess.2.Retrospective cohort study:BMI,neutrophil percentage was positively associated with abscess recurrence.Yin deficiency syndrome,overweight(BMI>25kg/m~2)were risk factors for recurrence of abscess.One-time radical operation is the protective factor for the recurrence of the abscess.BMI and diabetes,age of 40,had an interaction with the postoperative recurrence of the abscess.The non-linear relationship between BMI and the postoperative recurrence of abscess was found by curve fitting analysis of the GAM model.Conclusion:After systematic evaluation,20<age<40(years),poor lifestyle,comorbidities,Crohn's disease,diabetes(Chinese population),previous history of abscess,intersphincteric abscess,ischiorectal abscess,intestinal flora,and Incision and drainage were risk factors for recurrence;male,time from disease onset to surgery?7 days,Charlson comorbidity index 1-3 were protective factors.A cohort study showed that overweight,Yin deficiency syndrome,neutrophil percentage were risk factors for recurrence of abscess,and one-time radical operation was a protective factor,which was helpful for early identification of high-risk patients.
