Allergic asthma,cough variant asthma,and allergic rhinitis are allergic diseases that occur in the respiratory tract.The onset and anatomy of the three have the same homology,the etiology and pathogenesis are similar,and the clinical manifestations overlap each other,so we call them collectively "the lung and related allergic diseases".Western medicine has achieved remarkable effect in controlling the attack of the disease,but with the long-term use of the medicine,the side effects are more and more obvious.Some patients have the phenomenon of early effective but long-term effect reduced,even invalid,and the long-term effect is not ideal.Traditional Chinese medicine has a unique advantage in the treatment of this disease,but for the three diseases,they are mostly treated separately,and there is no discussion on the common pathogenesis and the treatment of different diseases.This study will combine literature review,theoretical research,and clinical research with biological evidence to discuss the same treatment of different diseases of the lung and related allergic diseases based on the theory of "syndrome differentiation of viscera as a whole in which lung and spleen as the core".Abjective:1.To find out the current status of Chinese and Western medicine research on lung and related allergic diseases,analyze the problems of Chinese and Western medicine in the treatment of this disease,and provide useful references for the research on the same treatment of different diseases through literature review.2.To expound and discuss the theory of "syndrome differentiation of viscera as a whole in which lung and spleen as the core",and the treatment of different diseases based on this theory through theoretical research.3.A variety of data mining methods to mine the common pathogenesis and clinical characteristics of lung and related allergic diseases to proved that it is a clinical demand to treat different diseases with the same treatment for lung and related allergic diseases,and it is feasible to treat different diseases with the same treatment for lung and related allergic diseases based on the theory of" syndrome differentiation of viscera as a whole in which lung and spleen as the core "through clinical research.4.To prove the effectiveness and reliability of "treating different diseases with the same treatment" of lung and related allergic diseases based on the theory of "syndrome differentiation of viscera as a whole in which lung and spleen as the core "through the experimental research.Method:1.Theoretical research:Combining the theory of traditional Chinese medicine and clinical practice to combing innovative ideas of same treatment for different diseases of lung and related allergic diseases based on the theory of "syndrome differentiation of viscera as a whole in which lung and spleen as the core ".2.Clinical research:A prospective study was conducted to collect the basic information,symptoms,tongue and pulse information of 349 patients with lung and related allergic diseases who met the inclusion criteria.The distribution of symptoms of lung and related allergic diseases was summarized by descriptive statistical analysis method.The clinical manifestations and the law of pathogenesis evolution of lung and related allergic diseases were explored by means of component ratio analysis,such as factor analysis,cluster analysis and other statistical methods.3.Experimental study:Sixty SPF BALB/c mice,half male and half female,were randomly divided into normal group,model group,dexamethasone group,high,medium and low dose groups of traditional Chinese medicine,with 10 mice in each group.The allergic asthma mice model was established and intervened by wenrunxinjinpei prescription.After the administration,the arterial blood,lung tissue,lung tissue homogenate,bronchoalveolar lavage fluid and other specimens were prepared.The levels of serum IgE,IL-4,IL-5,IL-17a and TNF-α in lung tissue were detected by ELISA,and EOS was counted in BALF.Result:1.Clinical research:In addition to the main symptoms,the clinical characteristics of the patients with lung and related allergic diseases are mainly dry mouth,bitter taste,dizziness,tinnitus,fatigue,sweating,chilly,less appetite,abdominal distention,frequent night urination,poor sleep,dry stool,loose stool and other symptoms of lung and spleen,and the deficiency and failure of other organs,phlegm or heat,are the syndrome of deficiency and excess;from the perspective of tongue vein,the most common tongue color of the patients is light red,dark red,light dark,and big fat tongue with teeth marks;tongue coating are mainly thin white coating and yellow greasy coating;pulse is most common in fine veins,followed by deep veins.Through factor analysis of 33 common symptoms of lung and related allergic diseases,11 common factors can objectively reflect the clinical characteristics of lung and related allergic diseases;analysis of 11 common factors can reflect the core pathogenesis characteristics of lung and related allergic diseases with lung and spleen as the core.Through cluster analysis,the common symptoms of lung and related allergic diseases were divided into 7 categories,and tongue pulse into 3 categories.It is suggested that the syndrome types of lung and related allergic diseases may include deficiency of lung Wei,deficiency of lung qi,deficiency of both lung and spleen,deficiency of spleen and kidney yang,turbid phlegm,phlegm heat and stagnation of lung and stomach heat,which embodies the core pathogenesis characteristics of multi visceral deficiency with lung and spleen as the core,as well as the pathological essence of the deficiency and the complex cold and heat.2.Experimental study:General situation:No animal died in the whole experiment process.After ova atomization attack,the mice in the model group had shortness of breath,irritability,scratching nose,face and nasal secretion.With the increase of atomization times,some mice showed mental depression,curling up,emaciation,and increased defecation and urination.All the symptoms were alleviated in dexamethasone group and each traditional Chinese medicine groups.Pulmonary function:When the concentration of methacholine was 3.125mg/ml,12.5mg/ml,50mg/ml,the airway resistance of mice in the model group increased(P<0.01),and the airway resistance was positively correlated with the concentration of methacholine.Compared with the model group,the airway resistance of the low and middle dose groups decreased at the concentration of 3.125 mg/ml,12.5 mg/ml and 50 mg/ml(P<0.05or0.01),while the airway resistance of high-dose group decreased at 12.5 mg/ml and 50 mg/ml(P<0.05).HE staining:In the normal group,the alveoli were basically in normal state,the wall was not thickened,the lumen was not dilated and secreting,the bronchial epithelium was intact,and no inflammatory cell infiltration was found.In the model group,the bronchial wall was thickened,the lumen was dilated and secreted,necrotic bronchial mucosal epithelium,muscular hyperplasia,inflammatory cell infiltration and structural disorder were observed.In dexamethasone acetate group,bronchial smooth muscle hyperplasia was not obvious,a small number of epithelial cells degenerated,and inflammatory cell infiltration was occasionally seen.In the traditional Chinese medicine group,few inflammatory cells infiltrated into the bronchial wall,no thickening of alveolar wall and mild hyperplasia of smooth muscle.EOS count in BALF:Compared with normal group,EOS count in BALF of model group was increased(P<0.01).Compared with the model group,EOS count in BALF of mice in low,middle and high dose groups of traditional Chinese medicine and dexamethasone acetate group were significantly decreased(P<0.05 or 0.01).Serum IgE:The level of serum IgE in model group increased(P<0.05).Compared with the model group,the serum IgE level of dexamethasone acetate group and middle dose group of traditional Chinese medicine decreased(P<0.05 or 0.01);the low and high dose groups of traditional Chinese medicine had a tendency to reduce IgE,but there was no statistical significance(P>0.05)The levels of IL-4,IL-5,IL-17a and TNF-α in lung tissue were as followsIL-4:The level of IL-4 in lung tissue of mice in model group was increased(P<0.01);compared with model group,the level of IL-4 in lung tissue of mice in dexamethasone acetate group and middle dose group of traditional Chinese medicine was decreased(P<0.01),while IL-4 in lung tissue of mice in low and high dose groups of traditional Chinese medicine showed a downward trend,but there was no statistical significance(P>0.05).IL-5:Compared with the normal group,the level of IL-5 in the lung tissue of the model group was increased(P<0.01);compared with the model group,the level of IL-5 in the dexamethasone group and the middle dose group of traditional Chinese medicine was decreased(P<0.05 or 0.01),and the level of IL-5 in the lung tissue of mice in the low and high dose groups of traditional Chinese medicine decreased,but there was no statistical significance(P>0.05);compared with the dexamethasone acetate group,the level of IL-5 in the middle dose group of Chinese medicine was decreased significantly(P<0.05)。IL-17a:The level of IL-17a in lung tissue of mice in model group was increased(P<0.01);compared with model group,IL-17a in lung tissue of dexamethasone acetate group and middle dose group of traditional Chinese medicine decreased significantly(P<0.01),while IL-17a in low and high dose groups of traditional Chinese medicine had a downward trend,but there was no statistical significance(P>0.05).TNF-α:the level of TNF-α in lung tissue of mice in model group increased(P<0.01);compared with model group,TNF-α level of mice in dexamethasone acetate group and middle dose group of traditional Chinese medicine decreased significantly(P<0.01),while TNF-α in low and high dose groups of traditional Chinese medicine decreased,but there was no statistical significance(P>0.05).Conclusion:1.The theory of "syndrome differentiation of viscera as a whole in which lung and spleen as the core" is rooted in clinical practice.Adhering to the thinking of seeking causes and treating diseases based on syndrome differentiation,following the overall concept,conforming to the needs of Zang Fu organs,and restoring the function of Zang Fu organs are pioneering innovation in the idea of "same treatment for different diseases "for lung and related allergic diseases2.The manifestations of lung and related allergic diseases are the deficiency of multi visceral functions with lung and spleen as the core,which is the syndrome of deficiency and excess,cold and heat intermingled;it is feasible to treat different diseases in the same way according to clinical practice,and it is feasible to treat lung and related allergic diseases based on the theory of "syndrome differentiation of viscera as a whole in which lung and spleen as the core".3.Wenrunxinjinpei prescription on the theory of "syndrome differentiation of viscera as a whole in which lung and spleen as the core" can effectively reduce the allergic symptoms of the animal models of allergic asthma,cough variant asthma and allergic rhinitis,reduce the allergic reaction,reduce the inflammatory damage,correct the immune imbalance,achieving the thought of "same treatment for different diseases" for lung and related allergic diseases,and proving the effectiveness and reliability of the thought of "same treatment for different diseases" for lung and related allergic diseases based on the theory of "syndrome differentiation of viscera as a whole in which lung and spleen as the core".
