| Objective to study the relationship between the TCM syndrome types of chronic glomerulonephritis after taking hormone and immunosuppressive drugs and the subgroup of lymphocyte,and professor Wang Gang's current academic thought and experience of syndrome differentiation and treatment in these patients,so as to better guide the clinical use of Chinese medicine and syndrome differentiation.Method 1.Sorting out 441 cases of chronic glomerulonephritis patients in the nephrology ward of Nanjing Boda Nephrology Hospital from 2011 to 2019 treated by Professor Wang Gang,according to the use of different immunosuppressive agents and different syndrome types,to observe the correlation between different immunosuppressive group and different syndrome type group and lymphocyte subgroup,the data of each group were calculated by SAS 9.4 statistical analysis software,the mean ± standard deviation was used in each group,the t test was used in the group comparison,and the changes of normal and abnormal values before and after treatment were described by cross-classification.P<0.05 was statistically significant.2.The medical records of Professor Wang Gang from January 2011 to June 2019 in the Kidney Hospital of Boda,Nanjing were systematically arranged,the information of the selected patient's name,sex,age,symptom,tongue coating,pulse condition,pathogenesis,treatment,prescription and medication were input into the data mining system of Xminer Operation Tool V1.4,SPSS 20.0 and SPSS Modeler 18.0,respectively,the data operation models of frequency distribution,association rules and cluster analysis are used to make statistics,analysis,induction and research on Professor Wang Gang's dialectical medication law.Result 1.Among the 441 cases studied in the analysis,there was a statistically significant change in the cd4cd8 values of the lymphocyte subpopulation 1 month after chronic glomerulonephritis treatment with hormones and immunosuppressive agents,these results suggest that the use of immunosuppressants may have an effect on humoral immunity,especially cd4cd8 in lymphocyte.At the same time,among the different syndrome types,t helper cells,CD4/CD8 and t suppressor cells were the lowest and the highest in qi-yin Deficiency Syndrome,taking different combinations of immunosuppressants had the most significant effect on the chronic glomerulonephritis syndrome.Among the different combinations of immunosuppressants,the hormone plus Tacrolimus and Tripterygium Wilfordii had the most significant effect on the lymphocyte.2.From January 2011 to June 2019,a total of 221 patients who were treated by Professor Wang Gang in Nanjing Boda nephrology hospital were selected for data mining research,and 381 medical records were included.Professor Wang Gang made a total of 147 visits(38.32%)to female patients and 234 visits(61.42%)to male patients in the treatment of nephritis complicated with hypoimmunity.All the patients aged from 3 to 80 were susceptible,the youngest age was 3 years and the oldest age was 81 years,especially among the young people aged from 30 to 39 years,accounting for 66.36%Clinical symptoms to foam urine,edema,waist ridge pain,Na Dai,tiredness and weakness,dry mouth pharynx,sore throat,cough,hand and foot heat-based,Pale red tongue,thin white tongue or red tongue less fur,thin and heavy pulse.Professor Wang Gang is good at tonifying Qi and nourishing Yin,clearing heat and removing dampness.He is also good at tonifying spleen and kidney,promoting diuresis and DETUMESCENCE,promoting blood circulation and removing blood stasis,removing dampness and removing turbid,nourishing liver and kidney,warming spleen and kidney,etc.,like to use PLANTAGO ASIATICA,tuckahoe skin,system stiff silkworm,Hedyotis Diffusa,whole Scorpion,Dogwood,Golden Cherry,yellow hollyhock flower,eupatorium,scutellaria.Through cluster analysis,three groups of core formula:Fangji Huangqi and Liu Wei Di Huang Wan,rhinoceros horn rehmannia soup,zhuling soup and Xiefeng soup.Conclusion 1.Based on the domestic and international literature,the latest domestic and international research progress of Chinese medicine,Western medicine,and the results of the correlation analysis between patients with chronic glomerulonephritis and weakened immunity and lymphocyte subsets in this study,the lymphocyte subset indicators are in the uptake of hormones.In patients with immunosuppressants,there are differences before and after use,and the results confirmed that the use of hormones and immunosuppressants can cause confusion in lymphocyte subsets.And it is proposed that lymphocyte subsets can be used as a routine and regular monitoring index to prevent infection.You need to be more cautious when using hormones plus tacrolimus and tripterygium wilfordii at the same time.2.Through the analysis in this data mining(frequency in Table 3-5,Table 3-6 and Table 3-7,Table 3-8),the instructor was transformed into patients with chronic glomerulonephritis using hormones and immunosuppressants The main symptoms of the syndrome are foamy urine,edema,back sinking.The main pathogenesis of clinical syndrome is the deficiency of both qi and yin,and according to the internal association rules of data mining pathogenesis,Table 3-12,the syndrome is most closely related to the internal accumulation of damp-heat and internal accumulation of blood stasis.3.According to the results of the analysis of the correlation between the Qi and Yin deficiency syndrome and lymphocyte subsets,(see Table 2-6 and Table 2-7),it is confirmed that the lymphocyte subsets index changes in the Qi and Yin deficiency syndrome are the most prominent,with T Helper cells,CD4/CD8 have the lowest values,and T suppressor cells have the highest values.The scope of the tutor's essential research on the deficiency of both Qi and Yin has been increased,and another objective indicator of deficiency of both Qi and Yin has been provided,namely:indicators of deficiency of both Qi and Yin and lymphocyte subsets in the body after taking hormones and only used a large sample of data to excavate the teacher's theory,law and prescriptions in patients with chronic glomerulonephritis and weakened immunity,but also passed a typical individual medical case to further demonstrate the treatment of replenishing qi,nourishing yin and clearing damp-heat Dafa,along with the syndrome,with the addition and subtraction of the syndrome,etc.,are characteristic medication rules,experience and clinical efficacy in the treatment of the disease.6.This study concluded that Professor Wang Gang's experience in the treatment of chronic glomerulonephritis is based on the treatment of the spleen,the liver,the lung,the pharynx,and the turbidity-relief and turbidity treatment. |
