Professor Song Xinwei's Academic Characteristics And Clinical Observation Of Ankylosing Spondylitis

Professor SONG Xin-wei engaged in the traditional Chinese medicine clinical,teaching and scientific research for more than 30 years,he has the unique insights and ideas about the treatment of ankylosing spondylitis,and the clinical curative effect is distinct.In this paper,I will show the summarized research about my tutor's academic ideas and treating experience of ankylosing spondylitis.First,Professor SONG Xin-wei's understanding about the disease name of ankylosing spondylitis in traditional Chinese medicine,he thinks the description of "da lv","shen bi","ji qiang","bei lv" in the ancient medical books was consistent with ankylosing spondylitis.Second,He thinks that the basic pathogenesis of ankylosing spondylitis is the inadequate gene,the deficiency of liver and kidney,with the invasion of the six evils,attack on the body,the evils stopping the Qi and blood circulation,so the phlegm turbid and blood stasis appear and seep into the bone,spine,eventually cause spinal stiffness and hunchback.He divides the disease process into early,middle and late periods.Each period can be divided into the activity and stability stages.The disease in activity stage is mainly controlled by hot and humid evils,so treatment is given priority to clear away heat and remove dampness,promote blood circulate and meridians and collaterals.Meanwhile invigorate the circulation of liver and kidney.The disease in stability stage is the deficiency of liver and kidney,this period of treatment should be based on benefit hepatorenal,and eliminate the blood stasis and turbidity.Finally,Professor SONG Xin-wei claims that Physician should grasp the core mechanism in the process of diagnosis,and the treatment is appropriate to collect the complex method.Treatment for activating blood circulation has always been.The lager dose of astragalus membranaceus to tonify Qi is necessary,then Rattan drugs is used to dredging collaterals,and insect drugs is used to relieve pain,He is good at using medication for compatibility,in order to achieve the purpose of synergies.He also claims on the basis of syndrome differentiation with traditional Chinese medicine drugs,choose some targeted therapeutic drugs confirmed by modern pharmacological research,in order to improve the clinical curative effect.Objective:Compared the efficacy and safety between the "gu bi NO.1" with compound betamethasone and Etanercept treatment on the phlegm and blood stasis type ankylosing spondylitis.Methods:60 patients with phlegm and blood stasis type ankylosing spondylitis were randomly divided into two groups:treatment group,30 cases treated by the "gu bi NO.1" with compound betamethasone therapy,control group,30 cases treated by Etanercept,Compare them with spinal pain VAS score,disease activity index(BASDAI)Bath,Bath disease functional index(BASFI),CRP,ESR and the evaluation of the TCM symptom integral scale,in order to evaluate its effectiveness.Results:1.The improvement rate patients achieved ASAS 20(50)between the treatmentgroup and the control group was similar in 2 weeks,12 weeks after treatment,so was the improvement rate of BASDAI 50.2.The treatment group got the total effective rate 89.7%,and the control group got 70.0%with TCM syndrome curative effect for 12 weeks,respectively,the difference was statistically significant.3.Both therapies can improve patients with spinal pain VAS score,morning stiffness time,pillow wall distance,thoracic mobility,refers to the distance,the modified Schober test,BASDAI and BASFI integral.4.Both therapies can reduce inflammation index the ESR and CRP significantly.5.Treatment group has good security than control group.Conclusion:After the treatment of 12 weeks,the treatment group was similar to the control group of ASAS 20(50)and BASDAI 50 improve levels,but the proportion of the syndrome of TCM effectiveness in treatment group was better than the control group.This showed that the "gu bi NO.1" with compound betamethasone therapy's curative effect in phlegm and blood stasis type ankylosing spondylitis is similar to the Etanercept treatment,but it was no obvious adverse reaction in the process of treatment,and it has economic material benefit and cost.This provides a reliable evidence for the clinical promotion of the therapy.
