| This thesis consists of three parts:the literature review,Meta-analysis and the clinical observation.The literature review is made up of three parts:One is traditional and western medicine research progress of cancer-related fatigue.From two aspects of Chinese medicine and western medicine to discuss the etiology,pathogenesis,and treatment of cancer-related fatigue.Another is basis and clinical application research progress of Jiaji points.It is discussd about the origin,the relationship with Jiaji point and main and collateral channels and zang-fu organs,prophylactico-therapeutic mechanism and clinical application of Jiaji points.The other is the research progress of cancer and T cell-mediated cellular immunity.It is summarized the relationship with cellular immunity and cancer-related fatigue.Meta-analysis is systematic assessmented about the effectiveness of external treatment of traditional Chinese medicine treatment of cancer-related fatigue and the relationship with cellular immunity and cancer-related fatigue.The clinical observation is observed the effectiveness of the ultrasound therapy drug penetration with Peiyuankangai Prescription treatment of cancer-related fatigue,and the relationship with cellular immunity and cancer-related fatigue.Objective:Through clinical observation,score the syndrome of cancer-related fatigue,quality of life and TCM syndrome,anylyze the effectiveness and safety of the treatment of patients with cancer-related fatigue by the ultrasound therapy drug penetration with Peiyuankangai Prescription which was based on literature and clinical observation.lt could provide new ideas and methods,explore the impact on the cytokine regulation,provide clinical service for the application of TCM for external use so as to conduct the further clinical promotion.Methods:Using prospective,multi-center study,propose diagnostic criteria,inclusion criteria and exclusion criteria.All observed cases were choosed from October 2014 to October 2016,oncology which were conformed to the standard of patients with cancer-related fatigue.They were randomly divided into 3 groups,the treantment group,the control group and blank group.All three groups were treated with chemotherapy,which was chosen the chemotherapy cycle for 21 days of chemotherapy,and the chemotherapy regimens are reference to the NCCN guidelines.The treatment group was treated with chemotherapy and the ultrasound therapy drug penetration with Peiyuankangai.The control group was treated with chemotherapy and the ultrasound therapy drug penetration with blank paster.The blank group was only treated with chemotherapy.The observation period was 42 days.To observe the clinical curative effect,quality of life,the changes of TCM syndrome and the change of cellular immune function,carries on the statistical processing.Result:According to the Piper fatigue rating scale,after treatment,the treatment group scores was significantly decreased.The difference was statistically significant(P<0.05).The control group scores was not seen the obvious change,and it was no statistically significant difference(P>0.05).The blank group scores was significantly increased.The difference was statistically significant(P<0.05).According to the BFI-C rating scale,after treatment,the treatment group scores were significantly decreased.The difference was statistically significant(P<0.05).The control group scores was not seen the obvious change,and it was no statistically significant difference(P>0.05).The blank group scores was significantly increased.The difference was statistically significant(P<0.05).According to the QLQ-30 rating scale,after treatment,the treatment group scores were significantly decreased.The difference was statistically significant(P<0.05).The control group scores was not seen the obvious change,and it was no statistically significant difference(P>0.05).The blank group scores was significantly increased.The difference was statistically significant(P<0.05).According to the clinical sumptoms of traditional Chinese medicine integral evaluation,after treatment,the treatment group scores were significantly decreased.The difference was statistically significant(P<0.05).The control group scores was not seen the obvious change,and it was no statistically significant difference(P>0.05).The blank group scores was significantly increased.The difference was statistically significant(P<0.05).According to security,there were no significant clinical changes in routine blood,liver and kidney function.Some patients had a mild skin reaction,which disappear with no treatment,considering no factors about the security.Conclusion:The ultrasound therapy drug penetration with Peiyuankangai Prescription can relieve cancer-related fatigue and improve the quality of life,improve the Qi and Yin deficiency symptoms and enhance immunity in cancer patients.In addition,by measuring blood CD3+ T lymphocytes cell count,CD4+ T lymphocytes cell count,CD8+ T lymphocytes cell count,CD4+/CD8+T lymphocytes cell ratio,and NK cell count,judge the state of cellular immune function,which relects the ultrasound therapy drug penetration with Peiyuankangai Prescription related to the regulation of T lymphocytes cell-mediated immune function.By adjusting the CD4+/CD8a T lymphocytes cell balance,regulate the body's immune function.At the same time,the fomula could relieve patients clinical symptom and improve the quality of life.From the perspective of traditional Chinese medicine,this formula could supplement Qi and nourish Yin,support healthy qi and remove pathogenic factors,Zang-fu functions unobstructed.And the patients' syndrome would relieve. |
