Purpose:To investigate the serum aldosteron elevel in patients with central serous chorioretinopathy(CSC).Methods:This is a combination of Cohort study and single arm clinical trial.Patients with CSC were included.All patients underwent fundus fluorescein angiography(FFA),enhanced depth imaging spectral-domain optical coherence tomography(EDI-OCT)and serum aldosterone assay.The patients were classified into spontaneously resolved and unresolved groups according to the 3-months follow up OCT examination.Recruit objects from unresolved group to treat with 40 mg spironolactoneorally for 2 months,and after the treatment,the EDI-OCT,best corrected visual acuity(BCVA),were tested again.Results:The study included 61 patients(72eyes)with 34 ones classified into the unresolved group.The aldosterone level was significantly associated with subfoveal choroidal thickness of involved CSC eyes(r=0.342,p<0.05),it was significantly associated with subfoveal choroidal thickness of uninvolved CSC eyes as well(r=0.348,p<0.05).And aldosterone level in the unsolved CSC group(161.8±50.1 ng/dl)was greater than it in the spontaneously resolved group(122.5±50.5ng/dl,p<0.05).The central macular thickness and SFCT were decreased significantly(p<0.05,p<0.05respectively),after treated with 40 mg/d spironolactone for two months.Conclusions:The unresolved CSC is characterized by high level of aldosterone,and it will cause SFCT thicked.Spironolactone treatment was associated with the improvement of chronic CSC,the side effect was rare. |
