Research backgroundLi Yuxian,born in Hutubi county,Xinjiang province,is a chief physician of TCM.He read quite a lot of traditional Chinese medical books treasured by his father and made up his mind to be a doctor when he was very young.He studied the skills of TCM from the famous Chinese traditional medical experts Yue Dexing and Zhang Xunbang successively.He has accumulated rich clinical experience in the treatment of internal medicine,gynecology,pediatrics,malignancy and other difficult miscellaneous multidisciplinary diseases in his 40 years of being a doctor.He co-authored the book,'Caoshantang Medical Inspection Record,' and has published more than 10 papers in Chinese traditional medical academic journals.Now he is a well-known doctor of TCM in Xinjiang province and was awarded the director of the academic inheritance of the Chinese fifth batch of the aged exceptional physician of TCM.Therefore,the unique system of academic thoughts and clinical experience requires students to excavate and utilize modern TCMdata processing techniques and means to find old Chinese medicine prescription of the structure and principle of extracting its main academic thinking,prescription characteristics etc.with the value of the content of the old Chinese medicine the academic thoughts and clinical experience to carry forward and passing the torch.Research purpose1.The author got the first hand clinical material during the three years clinical study under the instruction of Li Yuxian,.The author summarized the academic views that Mr.Li put forward in his clinical work and explored his academic sources by comparing them with the actual clinical cases.2.Then the author summarized all the conclusions analyzed by combining prescription metrology with TCM conventional theoretical methods.The clinical academic thoughts and experience collected are used to publish monographs of Mr.Li and some related research papers and will be improved and refined with modern medical theories and applied to the author's clinical work to further prove their clinical effect.To evaluate the clinical efficacy of'Huazhiwan' in the treatment of dyslipidemia(Pixutanzhuozheng),and further explore and reveal the mechanism of 'Huazhiwan' regulating blood lipid and anti vascular endothelial injury.Research contents and methods1.Theoretical researchWith the teacher by clinical interviews and data summarization method,inductive learning teacher Li Yuxian once dabbled in classical Chinese medicine,ancient physicians medical works,the essence part analysis,trace forming process of Li Yuxian's academic thought system,sort out the specific academic ideological origins.By learning with the teacher,interviews,analysis and summarize the diagnosis made by quantitative method of sorting out all related content mining,and summarize the formation of TCM clinical syndrome differentiation system of teacher Li Yuxian unique and academic ideological system,and sums up the common methods of clinical treatment prescription.2.Clinical researchThe objective of the clinical research is to observe how 'Huazhiwan',which have the effect of regulating blood lipid by enhancing bowel function and accelerating lipid metabolism,affect the following clinical indexes for patients with dyslipidemia:TC,TG,HDL-C,LDL-C,and the membrane markers CD62p,CD63 for platelet activation of cytokine,and the clinical safety through a standardized design of clinical trial.Then the efficacy and application period of 'Huazhiwan'and its therapeutic advantages and features are further researched to support its clinical application and promotion.A total of 87 dyslipidemia patients were enrolled in this study and randomly divided into two groups,cases that underwent 'Huazhiwan' therapy as HZW group and patients who were prescribed placebo as control group,both groups underwent Therapeutic Life-style Change.The HZW group were prescribed 'Huazhiwan,9g/time,2 times a day,half an hour before meals.While The control group were given the starch-based placebos that have the exact same dosage,size,color,weight and taste with 'Huazhiwan',and they were told to taken as the way the HZW group did.Each group take a course of 12 weeks.Various test data including clinical symptoms(syndromes),weight,body mass index,and safety monitoring indicators,clinical indicators collected before and after treatment of TC,TG,HDL,LDL,platelet activation of membrane markers CD62p,CD63 and clinical were recorded for efficacy,safety and mechanism assessment by visiting the subjects of both groups weekly after starting the experiment.Research resultsThe first part of this thesis introduces and summarizes the root sources of Li Yuxian's clinical academic thoughts.It explains how Mr.Li recommend that clinical therapy should take care not only the entire body but the spleen and stomach and emphasizes the basis of theory that all diseases comes from spleen and stomach.In the second part,the author further organized and summarized Mr.Li's clinical experience.Based on the deep understanding of the physiological function that spleen and stomach are tend to be damped and the understanding of the five main diseases,the author points out that all the dampness and swelling are the main basic pathology of spleen and stomach,so the ascending and descending of spleen and stomach should be noticed in clinical treatment.Besides,Mr.Li emphasizes the caring of spleen and stomach in clinical therapy,so all his clinical therapies started from the nursing of spleen and stomach whether it is tumor,heart disease,pulmonary disease or kidney disease.Buring the clinical treatment he often uses the therapeutic methods of Jianpihewei,Jianpihuashi,Jianpiyiqi and Huashihewei etc.Even after clinical treatment he also lay emphasis on the nursing of the functions of spleen and stomach.The author also summaries and refines his experience of his clinical study with Mr.Li.He concludes Mr.Li's four therapeutic skills and their identifications,medication rules and some distinguishing features.At the end,the author expanded the academic thoughts of Mr.Li,by illustrating the clinical treatment features of some miscellaneous diseases on the basis of Mr.Li's academic experience,such as the understanding of strengthening the spleen and stomach while eliminating the dampness with the changing of seasons,the application of the ten methods of curing vomiting while protecting the stomach and the analyzing of the therapeutic method of Jianpixiezhuo.The author also improves the analysis and conclusion of Mr.Li's clinical thoughts,medication and prescription rules.Mr.Li's personal clinical experience is mainly reflected in his prescriptions and the addition and subtraction of the herbs for different symptoms and their inheritance of the experience of the former generations of TCM experts.Hence,the most important part to inherit Mr.Li's clinical thoughts is to organize and analyze his clinical cases and to complete the key points for the cases,such as prescription,clinical history,symptoms,diagnosis,dialectics,legislation,commentary,etc.In addition,the clinical prescription is an important part of TCM inheritance.The special methods of treatment and the experience of medication can be obtained and the unique clinical academic thoughts can be refined through accumulating and analyzing Mr.Li's clinical prescriptions.In addition,Mr.Li's theory of the spleen and stomach should be emphasized in application and should be further explored and improved.The third part is mainly about the application of Mr.Li's clinical therapeutic experience that has been researched and sumarized.Mr.Li's representative prescription of the pills of'Huazhiwa' is researched in depth in this part to further explore its clinical significance;With the help of the modern technology,the effects of the clinical application of 'Huazhiwa' in relevant therapeutic areas is observed and researched.the treatment to the patients with dyslipidemia,enhancing liver,spleen,kidney,stomach,intestines and other entrails of performance while lowering diet,and its anti-Atherosclerosis role were also discussed in this part in depth.In recent years,the western medicine plays an important role with its rapid curative effect on curing dyslipidemia,but its side effects are gradually revealed as the study goes on.In addition,the development of the synthetic lipid-lowering new drugs are facing with great difficulties over the years,Besides,the TCM for lipid-lowering available in market is mostly the simple repetition of the experience and lack of any researches for its active parts and its effect and without any scientific theories to support.In this thesis the clinical features of 'Huazhiwa' is further studied in order to promote Mr.Li's clinical experience and make it serve more people.The author selects the most representative therapeutic way of strengthening spleen while purging turbid from Mr.Li's academic thoughts and clinical experience,then observes the clinical effect of 'Huazhiwan'in curing the dyslipidemia.Through analyzing the observation data,the clinical effect of Mr.Li's academic thoughts that strengthening spleen while eliminating dampness to treat the root causes of disease and purging turbid for relaxing bowels to cure the symptoms of disease is proved to be significant.The thesis discusses the effect mechanism of 'Huazhiwan',and states the significance of the application of Yitongweibu in the clinical therapy of dyslipidemia.The gender distribution of both groups is P>0.05,the difference was not statistically significant,balanced comparable;comparative age distribution P>0.05,the difference was not statistically significant,comparable equilibrium.Main outcome measures:? TCM syndrome.HZW group shows curative efficacy in improving TCM syndrome.the comparison of TCM syndrome effect of the subjects in two groups after treatment,P<0.01,a statistically significant difference.? lipid levels.'Huazhiwan'have effect in terms of regulation of blood lipids.comparison of lipid levels between subjects in both groups after treatment,P<0.01,the difference was statistically significant.Secondary outcomes:BMI comparison between two groups of patients after treatment,P<0.05,the difference was statistically significant.Discussion indicators:cytokine membrane markers of platelet activation CD62p,CD63,P<0.01,a significant difference.There were no obvious side effects found in the subjects of two groups in the course of treatment and no significant difference observed in safety evaluation indicators before and after treatment.Research conclusion1.Mr.Li's core academic thought is to 'protect taste' for the 'three' clinical treatment of cognitive system,based on the lifting of the spleen and stomach;the spleen and stomach is the basis of all things metaplasia;the spleen and stomach is the foundation of healthy prosperity.By emphasizing internal injurytaste,is riddled by students,and the key role in the treatment of spleen and stomach disease.2.Mr.Li in the diagnosis and treatment of Chinese medicine diseases,through systematic classification,special diseases,such as special methods to seize the main pathogenesis of the disease.On the basis of syndrome differentiation and treatment,emphasis is placed on treating diseases and taking the spleen and stomach as the key link,and Mr.Li's clinical experience of syndrome differentiation in spleen and stomach is reflected in his treatment of miscellaneous diseases and the forming of his therapeutic method of 'Jianpixiezhuo'.3.According to Professor Mr.Li's experience,'Huazhiwan' clinical observation,based on the current results of clinical studies,conclusion can be drawn that 'Huazhiwan' can improve the clinical syndromes as well as the quality of life for dyslipidemia patients and has an effect on controlling the BMI.It can also improve cardiopulmonary load,inhibit expression of platelet factor of endometrial cells,alleviate endothelial inflammation and regulate blood lipid.The clinical application of 'Huazhiwan' is proved to be safe and its curative effect is reliable.
