The Academic Thoughts And Clinical Experience Of Wang Shiqiang Professor (Attached: The Clinical Study Of The Synergism Of "Soothing Liver And Strengthening Spleen" In The Treatment Of Hypertension Through Tianma Gouteng Decoction)

Objectives:By analyzing teacher’s academic thoughts and treatment characteristics, summarizing down the academic thoughts and clinical experience especially the "Soothing Liver and Strengthening Spleen" of Professor Wang Shi qiang the fifth batch of national famous doctor of TCM so that the successor’s theories can be enlarged and the medical decision level can be improved. Wang Shi qiang professor,then try to find a new ideal for the treatment of hypertension by integrated traditional Chinese and Western medicine.Methods:By searching literature、learning TCM classics、analyzing teacher’s academic thoughts and treatment characteristics,then summarize down the academic thoughts and clinical experience of Professor Wang Shiqiang.On the other hand, design a clinical research about the synergism of Soothing Liver and Strengthening Spleen"in the treatment of hypertension of Yin Deficiency and Yang Hyperactivity through Tianma Gouteng Decoction by WangShiqiang professor,which based on the theory according to Professor Wang Shiqiang in the original view of hypertension:he believes that the relationship among hypertension^ liver and spleen are close.Normal liver function can help spleen to sustain the physiological function of ascending clear and descending turbid,namely"earth being dredged by wood".On the contrary, it will affectthe normal physiological function of spleen.LiverYin deficiency and peractive Yang;irregular flow of the liver qi,dys-splenism,phlegm and blo od stasis,disturb the upper orifices all of these end in hypertension.120 hypertension patients are included randomly.Then divide them into two groups namely treatment group and control group. And there are 60 cases in each group.After a course of treatment which contain 4 weeks,analyze the index as fllows:blood pressure、TCM symptom score、blood lipid Angiotensin Ⅱ、 aldosterone、C reactive protein among the patients to evaluate the effect.Results:The academic origin of Professor Wang Shiqiang,derive from classical learning:《Huang Di Nei Jing》、《synopsis of prescriptions of the Golden Chamber》、《Treatise on Febrile Diseases》、《the differentiation and treatment of epidemic febrile diseases》, and come from the learning from his own deceased father who is the famous doctor of TCM in SiChuan for many years. Then formed his own academic thought gradually:1.Emphasis the spleen and stomach, being good at Soothing Liver and Strengthening Spleen 1.2.Tonifying fire and helping yang to treat various kinds of disease.3.being good at treating diseases from SanJiao.By discussing and analyzing a large number of clinical cases try to summary the clinical thoughts of Professor Wang Shiqiang.1.the characters of syndrome differentiation: ① being good at treating diseases from SanJiao.②cure chronic cough by relieve shaoyang disorder.2.The characters of therapy:flexible and changeable according to pathogenesis:①Tonifying fire and helping yang to treat various kinds of cold disease.②Soothing Liver and Strengthening Spleen to treat different kinds of disease. ③ Regulating spleen and stomach to treat Pediatric diseases.3.Characteristics of TCM Prescriptions:CDbeing good at using of Classical Prescriptions; ② Multiple small prescriptions in a same prescription,the introduction of the experience of WangShiqiang Professor using Quadruple Combination Decoction to treat stomach disease. ③ being good at using basic prescriptions. ④ building new prescriptions by basing on pathogenesis.4.The characters of using drugs of TCM: ① Cough, asthma, phlegm often use Ginger、Asarum、 Schisandra chinensis。②Soothing Liver and Strengthening Spleen often use Bupleurum, Radix Paeoniae Alba, Radix Codonopsis, Atractylodes, poria.③Warming yang often use Ginger, Cinnamon, Aconite and Asarum. ④emphasizing opposite and complementary among different drugs of TCM.⑤tonifying the viscera must be going with tonifying kidney.5.The treatment of disease focus on the relationship between generation and restraint among five viscera.The results of the clinical study of the synergism of "Soothing Liver and Strengthening Spleen" in the treatment of hypertension through Tianma Gouteng Decoction:1.The differences of the therapeutic effects between the two groups have statistical significance.(P<0.05)2.The differences of the blood pressure before treatment between the two groups have no statistical significance. (P>0.05)The differences of the decreasing degree of blood pressure measured at 1、2 and 4 weeks after treatment of each group have statistical significance.But the differences of the decreasing degree of blood pressure measured at 4 weeks after treatment between the two groups have no statistical significance.( P< 0.05)3.The differences of the blood lipid between the two groups have no statistical significance.4.The differences of the Angiotensin II before treatment between the two groups have no statistical significance.(P> 0.05) The level of Angiotensin II decrease obviously after treatment in treatment group. (P< 0.05)The Angiotensin II decrease after treatment in control group.(P< 0.05)The differences of the Angiotensin II after treatment between the two groups have statistical significance.(P< 0.05)5.The differences of the aldosterone before treatment between the two groups have no statistical significance.(P>0.05) The level of aldosterone decrease obviously after treatment in treatment group. (P< 0.05)The level of aldosterone decrease after treatment in control group.(P< 0.05)The differences of the aldosterone after treatment between the two groups have statistical significance.(P< 0.05) 6.The differences of the CRP before treatment between the two groups have no statistical significance.(P> 0.05) The level of CRP decrease obviously after treatment in treatment group. (P< 0.05)The level of CRP decrease after treatment in control group.(P< 0.05)The differences of CRP after treatment between the two groups have statistical significance.(P< 0.05)Conclusions:WangShiqiang Professor’s characters of syndrome differentiation:"cautiously grasping pathogenesis"、"based on syndrome differentiation"、"combination of formula and syndrome ".The characters of therapy:flexible and changeable according to pathogenesis.Emphasis the relationship between generation and restraint among five viscera.These are all significant for the therapy.lt is confirmed that there exist the synergism of "Soothing Liver and Strengthening Spleen" in the treatment of hypertension by Tianma Gouteng Decoction,and exist the improving of general condition of patients.The mechanism may be related to the inhibition of the renin angiotensin aldosterone system and the decreasing the level of CRP in hypertension patients.The thoughts of WangShiqiang Professor in the treatment of hypertension are reliable and effective.
