| Objective:Take growing path and clinical cases of Professor He Huan-rong (Professor He Huan-rong, who’s the mentor of nation’s fifth group, and the first group in Jiangsu Province, and is responsible for training qualified and experienced Traditional Chinese Medicine (TCM) experts and the inheritance work of TCM) as foundation, to arrange and conclude Professor He’s road to success and academic origin; to summarize and analyze Professor He’s academic thoughts and clinical experience especially emphasizing on probing into syndrome differentiation regularity to pulmonary system diseases and medication experience which benefits in improving the curative effect of TCM, inheriting and developing valuable wealth of TCM and therefore is beneficial to patients. Moreover, by selecting Professor He’s clinical research on paste experience to observe clinical curative effect on patients in stable phase of COPD who are applied with He’s lung moisturizing paste combined with tiotropium bromide, hence, to explore the mechanism of this therapeutics on adjusting T lymphocyte subpopulations and related cytokine, and to provide the theoretical and experimental basis for the further development of Pharmaceuticals.Methods:Through the way that following the training mode which is to watch and learn from Professor He’s demonstration; to study from copied prescriptions which were issued by Professor He and participate in clinical exercises, etc. to explore and arrange Professor He’s road to success and academic origin and to conclude and analyze Professor He’s academic thoughts and clinical experience.In aspect of clinical research:The number of total case of patients in stable phase of insufficiency of QI of the lung and kidney COPD is 63, the number of amotio is 3. In which, the number of actual completed cases is 60 in which the number of paste group is 35 and the number of control group is 25. About control group:take in 18μg tiotropium bromide powders once every morning. Paste group:take He’s moisturizing lung paste 15g with water every morning and afternoon with an empty stomach based on previous mentioned therapy. After 6 month’s therapy, I assessed the number of acute attack, influenza, CAT scores, mMRC classification, pulmonary function, exercise tolerance (6MWT), and T-lymphocyte subsets and blood serum related cytokine from two groups of patients and compare with statistics taken before the treatment.Result:Professor He learnt from the master, study classics of TCM thoroughly, and devotes himself to academic research. As a doctor, Professor He is diligent and eager to learn to improve. Nevertheless, Professor He also attaches great importance to moral and culture accomplishment, focusing on scientific research and development of hospital for decades as one day. Excellence in both performing skills and moral integrity, Professor He has become an excellent doctor of his generation and he continues to work even after retirement in order to instruct and influence young doctors and to keep on inheritance work so to devoting himself for TCM. In aspect of Professor He’s main academic thoughts:(1) Differentiate Yin with Yang and know how to measure as a whole. Professor He emphasizes that holism is one of the important part of TCM characteristics, treatment based on syndrome differentiation is the quintessence of TCM and the examination of disease is based on distinguishing Yin from Yang. (2) The treatment to miscellaneous diseases should focus on kidney and spleen. In the process of treating many miscellaneous diseases, Professor He got good curative effect by diagnosis kidney and spleen along with reinforcing deficiency by harmonizing spleen-stomach and the ups-and-downs of qi movement. (3) Gyneco-diseases should focus on liver treatments. According to Ye Tianshi’s academic thoughts:"Liver being the innate basis of women" Professor He focus on nursing liver-qi, liver blood and removing heat from the liver, etc. when treating gyneco-diseases. (4) Differentiation and treatment of fever should analyze the actual situation. Professor He’s idea of treating fever due to yin deficiency, be careful with yin-fluid and eliminating heat by nourishing yin; selecting different methods to clear heat and remove toxicity in line with the difference with disease location and the nature of disease; reducing fever by reconciliation in cope with endogenous fever in case it’s intermingling of deficiency and excess; thinking that the root cause could be seasonal prevalent febrile pathogens, pathogenic dampness. So the treatment is to give consideration to two or more things. (5) Well-versed in both Chinese and western medicine, refer to both syndrome differentiation and disease differentiation. Professor He believes that we should take both advantages of TCM and western medicine, combine discrimination with disease diagnosis, and only in this way can we achieve better curative effect. In the aspect of Professor He’s academic experience about treating pulmonary system, this thesis respectively discusses clinical treatment on 5 common diseases:chronic cough, asthma, lung emphysema, lung cancer, lung withered along with each disease’s medical records.The result of clinical research:(1) The frequency of acute attack and influenza of both two groups has been reduced after treatment (P<0.05), in which the frequency of acute attack and influenza of paste group has been reduced obviously than that of control group (P<0.05). (2) CAT scores and mMRC classification of both two groups has been reduced after treatment (P<0.05, P<0.01), in which CAT scores and mMRC classification of paste group has been reduced more than that of control group (P<0.05). (3) The value of FEV1, occupied FEV1 predicted value%, FEV1/FVC% value after treatment have no obvious improvement (P>0.05); no statistic difference between control group after treatment than that of pre-treatment (P>0.05). (4) 6MWT of two groups of patients has been increased after treatment than that of pre-treatment (P<0.05, P<0.01), in which the result of paste group has more remarkable increase than that of control group (P<0.05). (5) After treatment the value of CD4+^ CD4+/CD8+for two groups has been increased than that of pre-treatment, significant difference being discovered from both groups(P<0.05), value of CD8+of paste group has been reduced(P<0.05); the value of CD4+、CD4+/CD8+is apparently higher than of control group(P<0.05). However, the value of CD8+after treatment has no significant difference between two groups (P> 0.05). (6) After treatment, the level of serum IL-8、IL-13、TGF-a has be decreased than that of pre-treatment (P<0.05, P<0.01), in which paste group is more obvious than that of control group which has significant difference by observing statistically (P<0.05). SOD of two groups has been increased than that of pre-treatment (P<0.05), in which paste group has more obvious increase than that of control group which has significant difference by observing statistically (P<0.05).Conclusion:Famous veteran teran doctors of TCM are outstanding delegate of today’s modern TCM academic and the highest level of clinical development. To arrange, investigate, and study national famous doctor of TCM Professor He’s academic thoughts and clinical experience is in accordance with the practical requirements of the modernization of TCM which is beneficial to the inheritance and development of TCM. The clinical research shows that applying He’s lung moisturizing paste combined with tiotropium bromide to patients in stable phase of COPD has better clinical therapeutic effect in improving patient’s acute exacerbation, influenza frequency, symptom sign, exercise tolerance than that of applying tiotropium bromide only. The mechanism of action could be associated with the adjustment of T-lymphocyte subsets and cytokine. |
