Section 1 Association study of BDNF rs6265 polymorphism with medication-overuse headacheBackgrounds and Objectives:MOH (medication-overuse headache) is a kind of relatively common headache, which may share the pathogenesis with drug dependant diseases such as heroin dependence. It is indicated that BDNF rs6265 polymorphism is associated with the development of drug dependant diseases, however, there is no research identifying the relation between BDNF rs6265 polymorphism and MOH occurrence. Clinical features of MOH were discussed in this study, in addition to the association between BDNF rs6265 polymorphism and MOH.Methods:Randomized block design was adopted in this research. Samples were collected according to 3 experiment groups:MOH group, migraine group, and healthy control group. Each group consisted of 120 cases with a male/female ratio of 1:3. Headache features of MOH group and migraines group were compared and medication overuse condition of MOH patients was analyzed. Gene types of BDNF rs6265 were detected by PCR using direct sequencing measurement and the genotype and allele frequency among MOH group, migraine group and normal control were compared respectively. We discussed the relationship of BDNF rs6265 polymorphism and MOH development, and the correlation of this polymorphism and medication consumption of MOH patients per month.Result:Compared with migraine group and healthy control group, MOH patients had relatively lower education background and income level. Combination analgesics were the most commonly overused medications. There was no statistics difference of genotypes and allele frequency among the three groups. MOH patients with non-GG genotype had a higher consumption of monthly drug number than those with GG. Among MOH patients who used "pain-stopping tablets" or "Paracetamol caffeine aspirin (PCA) powders", non-GG had a higher consumption of monthly drug number than GG patients; at multiple linear regression analysis, the BDNF rs6265 polymorphism emerged as a significant independent predictor of analgesic drug consumption.Conclusion:Polymorphism of BDNF rs6265 polymorphism might have no relation with the occurrence of MOH in Chinese Han population. While it may correlates to the medication consumption per month of MOH patients, non-GG patients had a higher consumption of monthly drug number than GG patients.Section 2 Headache in heroin dependent patients and the association of BDNF rs6265 polymorphism with heroin dependenceBackground and purpose:MOH has some similarities with drug dependence. Heroine dependence is the most common drug dependence in China. There are several researches on the relationship between BDNF rs6265 polymorphism and heroin dependence, however, the results are controversial. There is no research about headache in heroin dependent patients. This study observed the headache condition in heroin dependent patients, and the relationship of BDNF rs6265 polymorphism and heroine dependence.Method:120 cases of heroine-dependent patients are enrolled with a male/female ratio of 1 to 3. Investigation was conducted on the headache condition and features of heroine abusers. Gene type of BDNF Val66Met was identified through direct sequencing using PCR. Gene types of BDNF rs6265 were detected by PCR using direct sequencing measurement and the genotype and allele frequency among heroin group, MOH group and normal control were compared respectively. We discussed the relationship of BDNF rs6265 polymorphism and heroin dependence development, and the correlation of this polymorphism and heroin consumption of heroin dependent patients per month.Result:In heroin dependent patients, the prevalence of headache is rather high (90%), and 97.8% fulfilled the diagnostic criteria of "8.1.13 Headache attributed to use of or exposure to other substance". There was no statistics difference of genotypes and allele frequency among the three groups. Non-GG patients had a higher consumption of monthly heroin number than GG patients; at multiple linear regression analysis, the BDNF rs6265 polymorphism emerged as a significant independent predictor of monthly heroin consumption.Conclusion:Heroin-dependent patients have a rather high morbidity, most of whom fulfilled the diagnostic criteria for "8.1.13 Headache attributed to use of or exposure to other substance". Polymorphism of BDNF rs6265 polymorphism might have no relation with the occurrence of heroin dependence in Chinese Han population. While it may correlates to the heroin consumption per month of heroin dependent patients, non-GG patients had a higher consumption of monthly drug number than GG patients.
